自体脂肪颗粒注射隆乳术305例临床分析

摘　　要：目的：探讨自体脂肪颗粒注射隆乳术的效果和安全性。方法脂肪抽吸使用直径为3 mm的钝头三孔吸脂管连接20 ml 注射器。抽吸出的脂肪采用冷冻生理盐水冲洗,去除脂肪中混合的血液,然后用棉垫浓缩脂肪,分离其中的生理盐水、油以及破碎的脂肪细胞。乳房注射脂肪采用乳房下皱襞切口,切口长度约3 mm,注射管为直径14 G的单孔钝头管。脂肪注射入乳房后间隙至皮下层的多个层次,乳房腺体组织较为致密,使用钝头管可以避免将脂肪注入腺体。结果305例患者术后均Ⅰ期愈合。每侧乳房注射脂肪量120∽360 ml（平均226 ml）。126例获得随访6∽72个月,平均随访18个月。术后患者乳房形态、大小明显改善,乳房外形自然,质地均匀,手感较好。结论自体脂肪注射移植隆乳术是一种行之有效的隆乳方法。Objective To discuss the efficacy and safety of autologous fat transplantaion for breast aug-mentation. Methods Fat was harvested using 20 ml syringe attached to a three-hole blunt cannula with a di-ameter 3 mm. After washing with cool normal saline to remove blood, the fat was managed with open method u-sing cotton towel as a platform for concentration fat tissue and separating them from fluids, oil and debris. A 14-gauge, one-hole blunt cannula was used to place the fat through 3 mm incision on inframammary fold. The fat was infiltrated into the breast from deep to superficial subcutaneous plane. Using blunt cannula could prevent it from penetrating into the compact glandular tissue. Results 305 patients all obtained primary healing. Graf-ted fat volume has ranged from 120 to 360 ml （ average, 226 ml） per breast per session. 126 patients were fol-lowed 18 months （6 to 72 months）. Almost all women had a significant improvement in their breast size and shape postoperatively and the breasts were soft and natural in appearance. Conclusion Liposuction and autol-ogous fat transplantation is a suitable approach for augmentation mammaplasty.
