预防导尿管相关性尿路感染的环节质量控制

摘　　要：目的 通过监控促进留置导尿管维护环节质量改进,预防导尿管相关性尿路感染（catheter-associated urinary tract infection,CAUTI）。方法 2011-2014年,通过推行留置导尿管维护质量评价标准、操作规程和过程监控,促进维护环节质量改进;通过监测维护环节合格率和同期CAUTI感染率,评价环节质量改进对预防CAUTI的效果。结果 共监测2581例留置导尿患者。干预后维护质量合格率从2011年的32.66%提高至2014年的89.22%,同期CAUTI发病率从7.86例/千导管日下降至3.07例/千导管日。结论 通过监控留置尿管维护环节质量可以有效降低医院内CAUTI发病率。Objective To improve the quality of maintenance procedures on preventing catheter-associated urinary tract infection（CAUTI） by the targeted surveillance and interventions. Methods The targeted surveillance and interventions for the indwelling urinary catheter maintenance procedures were carried out from 2011 to 2014. The means included making and implementing the evaluation criteria and standard operation of maintenance procedures, evaluating the effectiveness of the procedures quality improvement on preventing CAUTI by comparing with the quality conformity rates and the incidence rate of CAUTI before and alter interventions. Results The maintenance procedures quality conformity rates increased from 32.66% to 89.22% and the rate of CAUTI declined from 7.86 per 1000 catheter days to 3.07 per 1000 catheter days. Conclusion The rate of CAUTI can be declined significantly by the targeted surveillance and interventions for the indwelling urinary catheter nmintenance procedures.
