针刺十七椎穴不同留针时间治疗原发性痛经的镇痛效果研究

摘　　要：目的:观察针刺十七椎穴不同留针时间治疗原发性痛经的镇痛效果,为确定最佳留针时间提供临床依据。方法:将符合纳入标准的90例原发性痛经患者,随机分为留针20 min组、留针30 min组和空白对照组,每组各30例。针刺组单刺十七椎穴,于月经第1天疼痛发作时针刺1次;空白对照组不予针刺治疗。应用视觉模拟评分法(VAS)和Mc Gill简化量表(SF-MPQ)观察各组治疗前后疼痛随时间变化情况,并进行比较。结果:针刺组镇痛效果从针刺10 min时开始显现,镇痛效果随留针时间的延长而增强;留针30 min组的镇痛效果显著优于留针20 min组(P<0.01);两针刺组均能明显改善SF-MPQ中各项指标的评分,留针30 min组显著优于留针20 min组(P<0.01)。结论:针刺十七椎穴对原发性痛经有明显的镇痛效果,留针30 min的镇痛效果优于留针20 min。Objective：To observe the analgesic effect of acupuncture on Shiqizhui（EX-B8） point with different needle retaining time,so as to provide reference for determining best needle retaining time in clinical treatment. Methods：Ninety patients with primary dysmenorrhea were randomly divided into 20-minute-needle-retaining group,30-minute-needle-retaining group and blank control group. Each group had 30 patients. The former two groups were given acupuncture on Shiqizhui（EX-B8） point on the first day of menstruation when intense pain occurred and the blank control group was not given acupuncture. The changes of scores of VAS and SF-MPQ before and after treatment of the three groups were observed and compared. Results ：The analgesic effect of the two acupuncture groups appeared after 10-minute needling and the effect enhanced over the needle retaining time;the analgesic effect of 30-minute-needle-retaining group was significantly better than that of the 20-minute-needle-retaining group（P〈0.01）;the scores of each index of the two acupuncture groups improved,but the improvement of the 30-minute-needle-retaining group was significantly better than that of the 20-minute-needle-retaining group（P〈0.01）. Conclusion：Acupuncture on Shiqizhui（EX-B8） point had significant analgesic effect on primary dysmenorrhea and the analgesic effect of 30-minute-needle-retaining was better than that of 20-minute-needle-retaining.
