摘 要:目的调查北京地区医院污水中含有NDM-1阳性的不动杆菌及耐药性情况。方法从北京多家医院采集污水标本,通过生化鉴定和16S rRNA测序进行病原分离鉴定;利用PCR技术对分离的病原进行bla NDM-1基因的鉴定;利用Sensititre药敏鉴定仪对NDM-1阳性菌进行抗生素敏感性分析。结果从医院污水中检出20株NDM-1阳性不动杆菌;所有菌对头孢类抗菌素、头孢西丁以及替卡西林和氨苄西林耐药率达到100%,有85%的菌对亚胺培南耐药。结论北京地区污水中产NDM-1阳性菌分离率较高,且主要为不动杆菌,其表现出多重耐药的特点,医院污水可能是NDM-1阳性菌存在的主要场所之一。Objective To investigate the prevalence of New Delhi Metallo-beta-lactamase-1 ( NDM-1 ) producing Acinetobacter isolated from sewage samples of hospitals in Beijing and to analyze its antibiotic resistance. Methods Sewage samples were collected from several comprehensive hospitals in Beijing and the isolated strains with suspicious resistance to imipenem were screened with biochemical identification and 16S rRNA sequencing. Polymerase chain reac- tion (PCR)technique was used to detect the blaNDM-1 gene from the isolated bacterial colonies. Antibiotic susceptibility testing of all the blaNDM-1 positive strains was performed by using the Sensititre instrument. Results Twenty blaNDM-1 positive Acinetobacter strains were detected, including 10 A. baumannii, 4 A. calcoaceticus, 4 A. lwoffi, and 2 A. junii. All strains were resistant to ceftriaxone, cefepime, cefepime, cefepime, cefoperazone, cefazolin, cefoxitin, and ampicillin and 85% of them to imipenem. Conclusion Isolation rate of the blaNDM-1 positive Acinetobacter in hospital sewage in Bei- jing is high and the blaNDM-1 positive bacteria are multidrug resistance. Hospital sewage may be one of main medium facilitating the emergence and spread of the blaNDM-1 positive bacteria.
