摘 要:目的:探讨复方蒿甲醚对伯氏疟原虫青蒿素抗性鼠疟的疗效。方法:伯氏疟原虫常规株经青蒿素逐量递增给药法连续培养16代,建立伯氏疟原虫青蒿素抗性株鼠疟模型。抗疟药效实验采用4d抑制给药法,采血镜检药后24h原虫血症密度、转阴率及28d治愈率。结果:经青蒿素逐量递增给药法连续培养16代后伯氏疟原虫青蒿素抗性株ED50为839.46mg/kg,抗性指数为3.33。青蒿素抗性鼠疟对本芴醇有可疑耐药(原虫血症密度>0.005);复方蒿甲醚对青蒿素抗性鼠疟药后24h的疗效优于本芴醇。复方蒿甲醚及蒿甲醚对青蒿素抗性鼠疟28d治愈率分别为70%和100%。青蒿素、本芴醇和磷酸氯喹分别对青蒿素抗性鼠疟有不同程度转阴,但都出现复燃,均未实现28d治愈。结论:青蒿素抗性鼠疟对本芴醇可疑耐药,可能是复方蒿甲醚对此抗性疟治疗后复燃的因素之一。Objective : To investigate the efficacy of artemether - lumefantrine (AL) in the treatment of artemisi- nin -resistant Plasmodium berghei malaria in mice. Methods:A conventional Plasmodium berghei strain was subjec- ted to increasing concentration of artemisinin for 16 serial passages and a mouse model of artemisinin - resistant Plasmodium berghei malaria was established. The efficacy of antimalarial drugs was determined by the standard 4 - day suppressive test. Parasitemia level and clearance rate at 24 hours after treatment and 28 - day cure rate were de- termined by microscopic examination of the blood samples collected. Results:After 16 serial passages with increas- ing concentration of artemisinin, the effective dose that reduced artemisinin - resistant Plasmodium berghei by 50% ( ED50 ) was 839. 46mg/kg and the resistance index was 3.33. Artemisinin - resistant malaria in mice had suspicious resistance to benflumetol (parasitemia level 〉 0. 005). The efficacy of AL at 24 hours after treatment was superior to that of benflumetol in treatment of artemisinin - resistant malaria in mice. The 28 - day cure rates of AL and ar- temether for artemisinin -resistant malaria in mice were 70% and 100%, respectively. For artemisinin -resistant ma- laria in mice, artemisinin, benflumetol, and chloroquine phosphate achieved different clearance rates;however, all of them failed to prevent relapse and cure malaria within 28 days. Conclusion :Artemisinin -resistant malaria in mice has suspicious resistance to benflumetol,which might be one of the factors for the relapse after treatment with AL.
