HLA-G基因14 bp插入/缺失多态性与原因不明复发性流产关联的Meta分析

摘　　要：目的探讨HLA-G基因14 bp插入（Insertion）/缺失（Deletion）多态性与原因不明复发性流产（recurrent spontaneous abortion,RSA）之间的关联性。方法检索万方、中国知网（CNKI）和Pub Med数据库,共检索出216篇文献,按统一标准筛选,最终纳入19项病例对照研究,采用Stata 11.0软件进行meta分析。结果本次研究共纳入RSA患者1 971人,正常对照1 861人。Meta分析结果显示：在总人群中,HLA-G 14 bp I/D多态性可能与RSA相关（ORI vs.D=1.157,95%CI1.1001~1.337,P=0.049,I2=60.2%;ORII＋ID vs.DD=1.299,95%CI 1.031~1.636,P=0.027,I2=54.2%）。排除不符合Hardy-Weinberg遗传平衡定律的文献后,结果显示,HLA-G 14 bp I/D多态性可能与RSA相关（ORII＋ID vs.DD=1.311,95%CI 1.058~1.895,P=0.019,I2=60.2%）。根据种族进行亚组分析后发现,在亚洲人群中,HLA-G 14 bp I/D多态性可能与RSA相关（ORII＋ID vs.DD=1.416,95%CI 1.050~1.688,P=0.016,I2=55.2%）,但未在欧洲人群中发现此现象。结论在亚洲人群中,HLA-G 14 bp I/D多态性可能与RSA存在关联性。Objective To explore the correlation between HLA- G 14 bp insertion/deletion polymorphism and unexplained recurrent spontaneous abortion（ URSA）. Methods A total of 216 related articles were retrieved from Wanfang database,CNKI and Pub Med database; according to the uniform criteria,19 articles on case- control study were finally selected which were available for meta- analysis using Stata 11. 0 software. Results A total of 1 971 RSA cases and 1 861 healthy controls were included in this study. Meta- analysis showed that in whole population,HLA- G 14 bp insertion / deletion polymorphism might correlate with RSA（ OR = 1. 157,95% CI： 1. 1001- 1. 337,P = 0. 049,I2= 60. 2%; OR = 1. 299,95% CI： 1. 031- 1. 636,P = 0. 027,I2= 54. 2%）. After excluding the articles that didn＇t consist with Hardy- Weinberg Genetic Balance Law,the results showed that HLA- G 14 bp insertion / deletion polymorphism might correlate with RSA（ OR = 1. 311,95% CI： 1. 058- 1. 895,P = 0. 019,I2= 60. 2%）. Population- based subgroup analysis revealed that in Asian group,HLA- G 14 bp insertion / deletion polymorphism might correlate with RSA（ OR = 1. 416,95% CI： 1. 050- 1. 688,P =0. 016,I2= 55. 2%）,however this was not found in European group. Conclusion HLA- G 14 bp insertion / deletion polymorphism may be correlated with RSA in Asian group.
