摘 要:目的:探讨血浆及胸水N-末端脑钠肽前体(NT-pro BNP)的相关性及其在胸腔积液中的诊断价值。方法:将2011年12月-2013年6月收治的胸腔积液患者90例分别检测胸腔积液及血浆NT-pro BNP浓度,研究血浆及胸水的NT-pro BNP浓度相关性;比较心源性胸腔积液组(A组)与非心源性胸腔积液组(B组)血浆及胸水NT-pro BNP的浓度;以胸腔积液NT-pro BNP浓度>1500 pg/ml,血浆NT-pro BNP浓度>1800 pg/ml为截点,计算胸腔积液及血浆NT-pro BNP水平诊断心源性胸腔积液的灵敏度、特异度及约登指数。结果:血浆与胸水NT-pro BNP水平呈正相关性(r=0.957,P<0.05);A组血浆及胸腔积液NT-pro BNP浓度均明显高于B组(P<0.05);以胸腔积液NT-pro BNP>1500 pg/ml为截点,其诊断心源性胸腔积液的敏感性和特异性及约登指数分别为:96.1%,87.8%,83.9%;以血浆NT-pro BNP>1800 pg/ml为截点,其诊断心源性胸腔积液的敏感性和特异性及约登指数分别为:97.1%,97.4%,94.5%。结论:血浆及胸水NT-pro BNP水平存在正相关性,对两者进行检测有助于心源性胸腔积液的诊断。Objective :To explore the relationship of NT-proBNP between plasma and hydrothorax and the diagnostic value in patients with pleural effusion. Methods : NT-proBNP in plasma and hydrothorax of 90 patients with pleural effusion were detected from Dec 2011 to Jun 2013 and the relationship was studied. Both the concentration of NT-proBNP were compared between cardiac pleural effusion group(group A) and no-cardiogenic pleural effusion group(B group). The sensitivity, specificity and Youden index of cardiac pleural effusion were calculated at the cutoff point of NT-proBNP in hydrothorax and plasma as above 1 500 pg / ml and 1800 pg / ml prospectively. Results :The NT-proBNP between plasma and hydrothorax had a positive correlation(r = 0.957,P〈0.05). Levels of NT-proBNP in plasma and hydrothorax in group A were significantly higher than those in group B(P〈0.05). The sensitivity,specificity and Youden index of the cut-off point as hydrothorax NT-proBNP 1500pg / ml and plasma NT-proBNP 1800 pg / ml of diagnosis of cardiac pleural effusion was 96.1%,87.8%,83.9% and 97.1%,97.4%,94.5%respectively. Conclusion : There is positive correlation of NT-proBNP levels between plasma and hydrothorax,testing NT-proBNP contributes to diagnosis of cardiac pleural effusion.
