摘 要:目的探讨儿童肺炎支原体(MP)感染早期心肌酶改变,对心肌损害的影响及早期干预治疗意义。方法对108例临床确诊为MP肺炎的患儿进行心肌酶谱、心电图及胸部X线检查分析,以了解支原体感染对心肌酶的影响。结果 108例MP肺炎患儿中,合并心肌酶改变者30例,占27%。胸闷、心悸、心前区不适均见于年长儿,其他病例缺乏心血管系统异常症状。30例均有2项或多项心肌酶增高,心肌肌酸激酶同工酶(CK—MB)均增高,心电图检查30例中异常24例(80.00%)。结论 MP肺炎可使心肌酶谱升高,尤其CK-MB升高为主要表现,其中症状体征不典型,客易漏诊,应密切注意患儿的症状及体征,对可疑病例应及早做心肌酶、心电图检查,以利早期诊断早期治疗,减少心肌损害的发生。Objective:To explore children’s mycoplasma pneumoniae(MP) infection in early myocardial enzyme changes, influence of myocardial injury and early intervention treatment. Methods:In 108 cases of clinical diagnosis of MP pneumonia with myocardial enzyme spectrum、Electrocardiogram(ecg) and chest X-ray analysis, To understand the mycoplasma infection of myocardial enzyme. Results: 108 cases of MP in children with pneumonia, merge myocardial enzyme changes of 30 cases, accounting for 27%. Chest tightness, palpitation, the area before the heart is unwell, are found in the older son, other cases lack of cardiovascular system abnormalities. 30 cases all have two or more of the myocardial enzyme increased, cardiac muscle creatine kinase isoenzyme(CK- MB) are increased, abnormal ecg examination in 30 cases of 24 cases(80.00%). Conclusion: MP pneumonia can make elevated myocardial enzymes, especially higher CK- MB as the main performance, in which symptoms and signs is not typical, fetal hydrops, should pay close attention to the patient’s symptoms and signs, for suspected cases should do myocardial enzymes and electrocardiogram examination as early as possible, and to facilitate early diagnosis of early treatment, reduce the occurrence of myocardial damage.
