摘  要:目的:观察腹腔暴露对大鼠肠黏膜屏障的影响。方法:将30只大鼠随机分为五组,即对照组、假手术组、腹腔暴露1 h组、腹腔暴露2 h组和腹腔暴露3 h组,每组6只。腹腔暴露组大鼠剖腹后暴露腹腔,但不进行任何手术操作,暴露相应时间后关腹。假手术组大鼠仅作腹壁切口,不打开腹腔。对照组大鼠仅作相应的麻醉。术后24 h,取血检测血清D-乳酸水平,并取回肠组织测定肠道FD4清除率、小肠黏膜紧密连接蛋白ZO-1和Occludin的表达水平,同时观察小肠组织的病理变化。结果:腹腔暴露组较对照组大鼠血清D-乳酸水平有所上升,肠道FD4清除率有所增加,小肠黏膜紧密连接蛋白ZO-1和Occludin的表达水平均有所下降,小肠黏膜病理形态有所改变,并且这种差异随腹腔暴露时间的延长而逐渐显著。Pearson相关性检验分析表明,腹腔暴露组大鼠血清D-乳酸水平和肠道FD4清除率与肠黏膜损伤Chiu's评分均呈显著的正相关。结论:腹腔暴露大鼠肠黏膜屏障功能受损,并随着腹腔暴露时间的延长而逐渐加重。Objective: To investigate the effect of peritoneal air exposure on intestinal mucosa barrier in rat. Methods: Thirty rats were randomized into five groups: a control group, a sham group, and three exposure groups with peritoneal air exposure for 1,2, and 3 h, respectively ( n = 6 each group). At 24 h after surgery, blood was obtained to detect the serum D-lactate, and the ileum tissue was sampled to determinate intestinal clearance of FD4, levels of tight junction proteins and histological changes. Results : Compared with the control group, groups of peritoneal air exposure showed an increase in serum D-lactate, intestinal clearance of FD4, and a decrease in levels of tight junction proteins. Different chan- ges were also found in the intestinal histological observation. These differences were proportion to the incremental time length of peritoneal air exposure. Furthermore, there was significantly positive correlation between serum D-lactate level and Chiu score for damage of intestinal mueosa using pearsoncorrelation analysis. The correlation between intestinal clearance of FIM and Chiu~ score was also shown significantly possitive. Conclusion : The data suggest that peritoneal air exposure could induce damage of intestinal mucosal barrier and this effect was proportional to the time length of peritoneal air exposure.
