摘　　要：目的 分析北京社区人群正常高值血压与肱踝动脉脉搏波传导速度（baPWV）的关系。方法 2007-2008年,在北京石景山地区两个社区（古城和苹果园社区）进行流行病学调查（n=3324）,入选资料齐全者2895人。调查血脂、血压、高敏C反应蛋白（hsCRP）和体质量指数（BMI）等相关心血管危险因素,测量baPWV。多因素Logistic回归分析调整相关危险因素后,以正常血压为对照,计算正常高值血压人群baPWV增高（≥14m/s）的比值比（OR值）。结果 随着血压水平升高,年龄、BMI、总胆固醇、三酰甘油、hsCRP和baPWV等增高。单因素Logistic回归分析显示,年龄、性别、高胆固醇血症、高三酰甘油血症、糖尿病、吸烟、BMI、hsCRP增高、正常高值血压和高血压均与baPWV增高相关。多因素Logistic回归分析显示,调整相关因素后,以正常血压为对照,正常高值血压人群baPWV增高的OR值为2.89（95%CI2.21~3.77,P〈0.01）。结论 baPWV增高与正常高值血压独立相关。Objective To analyze the relationship between high normal blood pressure （BP） and brachial-ankle pulse wave velocity in population of Beijing community. Methods In Gucheng community and Ping guoyuan community （Shijingshan District, Beijing）, 3324 subjects underwent the epidemiological survey from 2007 to 2008. 2895 cases whose data was complete were included. Lipids, BP, high-sensitivity C-reactive protein （ hsCRP ）, body mass index （BMI） and other related cardiovascular risk factors were measured, as well as brachial-ankle pulse wave veloci- ty （baPWV）. After adjusting for the associated cardiovascular risk factors, the odds ratio （OR） value of increased baPWV （≥14 m/s） in high normal BP subjects, compared to the normal BP subjects, was calculated with the multi- ple Logistic regression analysis. Results Age, BMI, cholesterol, triglyceride, hsCRP and baPWV rose with BP. The factors of age, gender, hypercholesterolemia, hypertriglyceridemia, diabetes, smoking, BMI, increased hsCRP, high normal BP and hypertension were associated with baPWV by univariate logistic regression analysis. After adjusting for the related risk factors, multivariate logistic regression analysis indicated that the OR value for the increased baPWV in high normal BP subjects was 2.89 （95% CI 2.21--3.77, P〈0.01）, as compared with nor- mal blood pressure group. Conclusion The increase of brachial-ankle pulse wave velocity is independently associat- ed with high normal BP.
