心理护理配合针刺改善中风患者肢体功能的临床评价

摘　　要：目的观察心理护理在中风后痉挛瘫痪患者肢体功能治疗中的作用。方法 45例中风痉挛瘫痪患者,随机分为心理护理组(23例)、常规治疗组(22例)。心理护理组为常规治疗组基础上增加心理护理治疗,常规治疗组采用常规针刺疗法,常规选穴合谷、曲池、肩髃、足三里等,在治疗前后观察患者肌力、肌张力、肌痉挛状态和关节活动度变化的情况。结果经过4周治疗,心理护理组总有效率为95.65%,优于常规治疗组86.36%(P<0.05)。与治疗前相比,两组患者肌力、肌张力、肌痉挛状态的评分和关节活动度均有所改善,心理护理组对症状改善的影响强于常规治疗组(P<0.05)。结论心理护理疗法的运用对改善脑中风痉挛瘫痪患者的肌力、肌张力、肌痉挛状态和关节活动度均有促进作用。Objective To observe the effect of psychological nursing in limb function of stroke patients with spastic paralysis. Methods Forty-five cases with spastic paralysis of stroke were randomly divided into psychological nursing group（ 23 cases） and control group（22 cases）. The psychological nursing group was treated with acupuncture therapy and psychotherapy. The control group was treated by acupuncture at Hegu（LI4）, Quchi（LI11）, Jianyu（LI15）, Zusanli（ST36） etc.Before and after treatment,the change of muscle strength, muscle tonus, muscle spasticity and joint activity were observed. Results After four weeks, the total effective rate of the psychological nursing group was 95.65% which was better than that of the conventional treatment group 86.36 %（P〈0.05）. Comparing with the situation before treatment, the muscle strength, muscle tension, muscle spasticity score and joint activities of the two groups were improved. The effect on symptom improvement of the psychological nursing group was stronger than that of the conventional treatment group（ P〈0.05）. Conclusion Application of psychological nursing therapy can improve muscle strength, muscle tension, muscle spasticity and joint activities of stroke patients with spastic paralysis.
