彩色多普勒超声对良、恶性胃间质瘤的鉴别诊断

摘　　要：目的探讨胃间质瘤(GST)良、恶性不同的彩色多普勒超声表现。方法回顾性分析2008年7月至2013年12月解放军第一八〇医院收治的经手术病理证实的良性GST(14例)及恶性GST(9例)患者的术前彩色多普勒超声表现,包括病灶形态大小、血流丰富程度、动脉血流峰值速度(Vmax)及血流阻力指数(RI)。结果 14例良性GST患者血流信号不丰富,RI为低阻型,9例恶性GST患者血流信号丰富,RI为高阻型。两者彩色血流信号丰富程度及血流RI比较差异有统计学意义(P<0.01),而Vmax比较差异无统计学意义(P>0.05)。结论彩色多普勒超声有助于良、恶性GST的鉴别诊断,可为临床提供更多的诊断信息。Objective To investigate the different features of benign and malignant of gastric stromal tumors （GST） in color Doppler ultrasound.Methods Retrospective analysis on preoperative color Doppler ultrasonography of benign gastric stromal tumor（14 cases） and malignant gastrointestinal stromal tumors （9 cases） confirmed by postoperative pathology in PLA 180th Hospital during Jul.2008 and Dec.2013 was done,including the shape and the size of the lesion,the degree of blood flow,arterial blood flow peak velocity （Vmax） and vascular resistance index（RI）.Results The 14 patients with benign tumors：GST blood flow signal not rich,RI weas low;the 9 patients with malignant tumors：GST blood flow signal was rich,high RI. Differences in richness of color blood signals and RI were statistically significant（P〈0.01）,while the differ-ence in Vmax was not significant（P〉0.05）.Conclusion Color Doppler ultrasound can help the differential diagnosis of benign and malignant GST,providing more diagnostic information.
