摘 要:目的:了解2011年、2012年我院感染鲍曼不动杆菌的临床分布及耐药性,为临床用药及耐药菌的防治提供依据。方法:收集我院2011年1月至2012年12月常规培养检出的306株鲍曼不动杆菌,分析其在临床标本和病房的分布、耐药状况。结果:检出306株鲍曼不动杆菌中,标本主要分布以痰液为主,占77.78%;科室分布主要为重症监护病房(ICU)最高,2011年占40.0%,2012年上升至60.9%,检出率增长迅速,其次为神经外科、呼吸内科、骨科分别占(2011年)23%、12%、9.0%,(2012年)9.71%、8.74%、3.88%;鲍曼不动杆菌对哌拉西林他唑巴坦的耐药率从2011年的68.0%上升至2012年的71.36%,对亚胺培南的耐药率从2011年的66.0%上升至2012年的72.81%,2012年对氨苄青霉素、头孢唑肟、头孢替坦、呋喃妥因耐药率均为100%。结论:鲍曼不动杆菌主要引起呼吸道感染,耐药性严重,多为多重耐药甚至泛耐药,且耐药性逐年上升趋势明显,应积极防治院内感染,合理使用抗生素、规范医疗操作,以减少耐药株的上升。Objective:To investigate the distribution and drug resistance of Acinetobacter baumannii isolated from year 2011 to 2012. To provide basis for clinical application of antibiotics andprevention and sotvement of drug-resistant.Methods :Totally 306 cas- es of Acinetohacter hanmannii cultured in normal methods from Jan 201 lto Dec 2012 were analyzed in the variability of clinical speci- mens,ward distribution and drug resistance.Results: In totally 306 cases of Acinetobacter baumannii,the specimens were mainly ob- tained from sputum,accounting for 77.78%.Acinetobacter baumannii was primary discovered in the Intensive care unit (ICU),account- ing for 40.0% in 2011,and 60.9% in 2012,with detection rate increased rapidly,sequensely with neurosurgery department,respiratory medicine and orthopedics,accounting for 23%,12% and 9.0% in 2011,9.71%,8.74% and 3.88% in 2012,The resistance of Acineto- bacter baumannii to piperacillin sodium was increased from 68.0% in 2011 to 71.36% in 2012.The resistance to imipenem was in- creased from 66.0% in 2011 to 72.81% in 2012. The resistance rate of Acinetobacter banmannii to ampicillin,cftizoxime,cefotetan,ni- trofurantion was 100% in 2012.Condusion: Acinetohacter baumannii leading to respiratory tract infection has seriousely antibiotic resistance, most of which was multiple drug resistance or extensive drug resistance,increasingly year by year.The prevention and control of nosocomial infection should be emphasized, with the antibiotics being reasonably used and the medical operation being properly taken,which can hault the speed of rising resistantance.
