患者参与质量控制降低 PICC 导管并发症发生率的效果

摘　　要：目的：探讨患者参与质量控制在减少 PICC 导管并发症发生率中的应用效果。方法将江苏省淮安市第一人民医院 PICC 导管维护标准设计成通俗易懂、患者专用的 PICC 导管护理质量控制单（简称质控单）,使 PICC 置管患者在住院期间可以按照质控单标准对护士 PICC 导管护理进行监督。选择2013年7—12月未参与 PICC 导管质控的 PICC 带管患者作为对照组,2014年1—6月参与 PICC 导管质控的 PICC 带管患者为观察组,比较两组 PICC 导管留置期间的并发症及患者满意度。结果观察组的 PICC 导管留置期间静脉炎发生率为6.3%,感染发生率为3.8%,堵管发生率为1.3%,血栓发生率为1.3%,均低于对照组,差异有统计学意义（χ^2值分别为4.139,4.981,3.888,0.377；P 〈0.05）；观察组患者满意度为97.5%,对照组为87.5%,差异有统计学意义（χ^2=5.645,P 〈0.05）。结论让患者参与PICC 导管维护的质控,使护士和患者都能积极参与 PICC 导管质量管理,加强患者安全意识,有效控制护理并发症的发生,提高了护理质量。Objective To explore the effect of application of patients′ participation in the quality control in reducing the complications of PICC catheter. Methods An easy-to-read, dedicated PICC nursing quality control list was designed to supervise the nursing intervention during hospitalization. The patients with PICC were selected to the control group, from July to December 2013. The patients with PICC were selected to the experiment group and participated to the quality control, from January to June 2014. The complications of PICC catheter and patients′ satisfaction were compared. Results The incidences of phlebitis, infection, blocking pipe and thrombosis in the experiment group were 6. 3% , 3. 8% , 1. 3% and 1. 3% , respectively, which were significantly lower than those in the control group （χ^2 = 4. 139,4. 981,3. 888,0. 377;P 〈 0. 05）. Patients′ satisfaction in the experiment group was 97. 5% , which was significantly higher than 87. 5% in the control group （χ^2 = 5. 645,P 〈 0. 05）. Conclusions The patients′ participation in quality control of PICC catheter can increase the cognition of patients′ safety, effectively control the complications and improve the nursing quality.
