摘  要:目的探讨不同稀释介质及方式对血清雌二醇浓度(estradiol,E2)的影响。方法采用化学发光免疫法进行血清雌二醇浓度测定,以60例仪器报告结果覆盖线性范围内外的患者血清作为样本,每例等分为若干份,分别使用厂商指定稀释液S0、低值血清和生理盐水作为稀释介质,进行手工1∶1及1∶4稀释后上机检测,换算最终浓度;同时每管均设3组平行管取其均值,并进行统计分析。结果用厂商指定稀释液S0、低值血清进行组间和组内不同比例稀释后雌二醇的最终浓度的差异无统计学意义(P>0.05),使用生理盐水进行不同比例稀释后雌二醇的最终浓度与原始值的差异有统计学意义(P<0.01)。结论低值血清可作为化学发光免疫法测定高浓度雌二醇的替代型稀释液,而生理盐水则不适宜作为化学发光免疫法测定高浓度雌二醇的稀释介质。Objective To evaluate the impact of different diluting media and modes on determination of high seral concentration of estradiol(E2).Methods 60 serum samples,in which E2 concentrations were determined by chemiluminescent immunoassay covering the linear range of the instrument limitation(UniCel DXI 800,Beckman,USA),were selected for the diluting experiment.Each sample was divided into several equal volumes and diluted into 1∶1and 1∶4with the media of specific diluent S0 provided by manufacturer,low-level E2 serum and normal sodium.The samples was measured with the instrument.The results were converted into final concentration,each tube was set with three parallel tubes,the average value was calculated,and statistically analyzed through one-way ANOVA.LSD t-test was applied to compare the difference in each group.Results The E2 concentration measured originally showed no statistical difference(P〉0.05)with those diluted by specific diluent S0 and low-level E2 serum at different diluting proportions.A significant difference was found between the E2 concentration in original group and the group diluted with normal saline at different proportions(P〈0.01).ConclusionsThe low-level E2 serum can serve as a replacement of diluent for the sample of high estradiol concentration when chemiluminescent immunoassay was used.Normal saline is not a medium suitable for dilution of the sample with high estradiol concentration.
