促排卵治疗中卵泡黄素化PCOS不孕患者血清甲状腺激素水平变化及意义

摘　　要：目的：观察促排卵治疗中卵泡黄素化多囊卵巢综合征（ PCOS）不孕患者血清甲状腺激素水平变化，并探讨其临床意义。方法对89例PCOS患者用促排卵药物（ CC／HCG）诱发排卵，其中卵泡直径至18～20 mm并正常排出者49例为对照组，卵泡未能排出、出现黄素化40例为黄素化组。检测两组不同卵泡生长时期血清甲状腺激素水平。结果卵泡直径至5、10、15 mm时，两组甲状腺激素水平差异无统计学意义；卵泡直径至20 mm时，黄素化组T3、T4、TSH水平分别为（1．97±0．17）nmol／L、（97．43±0．16）nmol／L、（3．43±0．10）μIU／mL，对照组分别为（2．69±0．15）nmol／L、（124．38±0．29）nmol／L、（1．38±0．18）μIU／mL，两组T3、T4、TSH水平相比，P均＜0．05。结论促排卵治疗中发生卵泡黄素化的PCOS不孕患者血清T3、T4水平下降，TSH水平升高，甲状腺激素变化可能与患者排卵功能异常有关。Objective To observe the changes in levels of thyroid hormone in patients with polycystic ovary syndrome （ PCOS） about luf of follicle during ovulation induction and to investigate the significance.Methods Eighty-nine patients with PCOS were induced ovulation by CC＋HCG.During menstrual cycle of induced ovulation, when the follicles grew to 18-20 mm and can be normally educed, we took these as the control group （49 cases） , and the other follicles failed to dis-charge, we took those as the luf group （40 cases）.The levels of thyroid hormone at different follicular development period were detected.Results The levels of thyroid hormone were not significantly different when the diameter of follicles grew to 5 mm, 10 mm and 15 mm during the follicular phase.When the follicles grew to 20mm size, the levels of T3 , T4 and TSH of luf follicle were respectively （1.97 ±0.17） nmol/L, （97.43 ±0.16） nmol/L and （3.43 ±0.10） uIU/mL in the luf group;They were （2.69 ±0.15） nmol/L, （124.38 ±0.29） nmol/L and （1.38 ±0.18） uIU/mL in the control group, and significant differences were found in the levels of T3 , T4 and TSH （ all P〈0.05） .Conclusion The levels of serum T3 and T4 in patients with PCOS about luf of follicle during the ovulation induction are decreased and TSH is in-creased, and the changes in levels of thyroid hormone may be related with the dysfunction of ovulation induction.
