来氟米特与环磷酰胺治疗狼疮性肾炎的对比研究

摘　　要：目的：探究来氟米特与环磷酰胺对狼疮性肾炎临床治疗的有效性、安全性以及临床上推广的可行性。方法对在2013年1月～2014年7月期间山东省济宁市第一人民医院收治的34例狼疮性肾炎患者按照入院时间随机将患者分成对照组和观察组，其中观察组患者17例，使用来氟米特治疗；对照组中患者17例，使用环磷酰胺治疗，6个月为1个疗程，在治疗的过程中观察记录患者在治疗前后肝肾功能指标和血清蛋白、24h尿蛋白定量、抗双链DNA抗体滴度等指标变化。在1个疗程结束后，对2组的狼疮基本活动指数进行评估。结果对照组治疗有效率70．5％，观察组有效率88．2％，2组差异不具有统计学意义；其余指标治疗前后差异都具有统计学意义（ P＜0．05）；对照组的不良反应发生率为42．9％，观察组的不良反应发生率为14．3％，2组差异具有统计学意义（ P＜0．05）。结论来氟米特对狼疮性肾炎的治疗有显著的效果且副作用小，安全性高。Objective To explore the clinical therapeutic effectiveness,safety and feasibility clinical of leflunomide and cyclophosphamide on lupus nephritis.Methods 34 cases with lupus nephritis were randomly divided into control group and observation group according to the time of admission during January 2013 to July 2014 in Shangdong Jining NO.1 People’s Hospital.17 cases in observation group were given leflunomide;17 cases in control group were given cyclophosphamide,six months was a period of treatment.Two groups were in the process of therapy observed and recorded changes in liver and lidney function indexes,serum albumin 24-hour urinary protein,anti-double stranded DNA antibody titers before and after treatment.At the end of a course, the basic activities of the two groups of lupus index were evaluation.Results The total effective rate in control group was 70.5%, and that was 88.2% in the observation group, there was no difference significant.Compared with the other indexes before and after treatment were statistically significance （P〈0.05）; the incidence of adverse events in the control group was 42.9%, the incidence of adverse events in the observation group was 14.3%, the difference was statistically significant （P〈0.05）.Conclusion Leflunomide in treatment of lupus nephritis has significant effect, less side effects and high safety.
