经皮与开放手术治疗胸腰椎骨折的疗效观察

摘　　要：目的观察经皮微创手术和开放手术治疗胸腰椎骨折患者的临床效果。方法将我院2012年2月至2015年1月期间收治的85例胸腰椎骨折患者按照手术治疗方法的不同分为微创组(45例,经皮微创手术)和开放组(40例开放手术),观察两组患者的手术治疗效果。结果术前术后两组患者的伤椎前缘高度、后凸Cobb角、矢状位指数比较存在的差异均无统计学意义(P>0.05),微创组患者术后1个月、术后3个月的日常生活能力(ADL)评分均明显高于开放组患者(P<0.05),术后1周和术后1个月的疼痛评分(VAS)均明显低于开放组患者(P<0.05),术后并发症发生率为2.2%,明显低于开放组患者的17.5%(P<0.05)。结论经皮微创手术治疗胸腰椎骨折患者,创伤小,疗效显著,安全性高,具有临床推广应用价值。ABSTRACT：Objective： observe clinical effect of percutaneous minimally invasive surgery and open surgical treatment for thoracolumbar fracture. Method：divide 85 cases thoracolumbar fracture patients received in our hosptial during 2012 February and Jan 2015 into minimally invasive group （45 cases, percutaneous minimally invasive surgery） and open group （40 cases, open surgery） according to surgical treatment methods. And observe clinical effects of both groups patients. Result：anterior height of vertebral body, Kyphosis Cobb angle, sagittal index of two groups before and after operation show differences, and comparison has no statistical significance （P 〉 0.05）. Patients’ ability of daily living （ADL） of minimally invasive group 1 month, 3 months after opeartion receives signiifcantly higher scores than those of open group （P〈0.05）, pain scores （VAS） 1 week and 1 month after operation is signiifcantly lower than those of open group （P〈0.05）, postoperative complications rate is 2.2%, which signiifcantly lower than that of open group of 17.5%（P〈0.05）. Conclusion：minimally invasive surgical treatment for thoracolumbar vertebral fracture shows advantages of small trauma, remarkable curative effect, safe, which has clinical value of being spreaded and applied.
