经腹腹腔镜腔静脉后输尿管吻合术治疗腔静脉后输尿管的安全性和疗效分析(附12例报告)

摘　　要：目的:探讨经腹腹腔镜腔静脉后输尿管吻合术(laparoscopic ureteroureterostomy,LUUS)治疗腔静脉后输尿管的手术方法、安全性及疗效。方法:回顾分析2010年10月至2014年10月为12例患者行LUUS的临床资料,其中男9例,女3例,平均(38.5±12.6)岁,体质指数平均(22.83±2.28)kg/m2。术前经彩超、静脉尿路造影、泌尿系CTU明确诊断。分析手术时间、术中出血量、手术成功率、术后住院时间、并发症发生率等指标。结果:12例手术均获成功,无中转开放手术。手术时间平均(156±58)min,术中出血量平均(105±53)ml,术后平均住院(9.5±3.7)d。术中、术后均无输血,无切口感染及漏尿发生。术后平均随访(33.8±16.7)个月,患者输尿管梗阻明显改善,随访期内无复发。结论:LUUS治疗腔静脉后输尿管安全、有效,手术创伤小,术后康复快,但手术时间较长。Objective：To explore the operative methods, safety and efficiency of transperitoneal laparoscopic ureteroureterostomy （LUUS） for the treatment of retrocaval ureter. Methods ：The clinical data of 12 patients with retrocaval ureter undergoing transperitoneal LUUS from Oct. 2010 to Oct. 2014 were reviewed retrospectively, including 9 males, 3 females. The average age was （ 38.5 ± 12.6） years old,and body mass index （BMI） was （22.83 ±2.28） kg/m2. Before operation,the 12 patients were diagnosed by B-uhrasound, IVU and CTU. Data on the operation time, the blood loss during operation, the operation successful rate, the mean hospitalization time after operation,the complications were analyzed. Results： All 12 patients were successfully operated, no cases were converted to open surgery. The operation time was （156 ± 58 ） min, the blood loss during operation was （105± 53 ） ml, the hospitalization time after operation was （9.5 ± 3.7） d. No blood transfusion, wound infection or leakage of urine occurred. A mean follow-up was conducted for （33.8 ±16.7） months. Remarkable improvement in the ureteral obstruction was observed, and no recurrence of ureteral stricture was found. Conclusions ： Transperitoneal LUUS is safe, efficient, mini-invasive with fast recovery and long operative time for the retrocaval ureter. It is worthy to be applied clinically.
