胃扭转对婴儿巨细胞病毒感染胆汁淤积症的影响及对策

摘　　要：目的:探讨巨细胞病毒(CMV)感染胆汁淤积症合并胃扭转或/和肺炎患儿,在抗病毒的同时,按胆汁淤积的不同程度给予中药治疗,并结合手法按摩及时整复胃扭转,观察病程中发生肺炎及严重程度和对预后的影响。方法:选取120例本院住院1-6个月同时符合婴儿CMV感染胆汁淤积症、胃扭转诊断患儿,根据胆汁淤积程度分为常见证型组、重型组,各60例,两组再随机分为对照1、2组和治疗1、2组,各30例。按观察指标记录并测定观察治疗前后各组患儿体征及实验室监测结果。评价治疗后病程中发生肺炎及严重程度,以及3种疾病的临床疗效和预后。结果:治疗后,各治疗组CMV感染胆汁淤积症、胃扭转、肺炎的临床治愈率均好于各对照组(P<0.05,P<0.01);治疗组病程中发生肺炎及严重程度显著低于对照组(P<0.05,P<0.01)。结论:婴儿CMV感染胆汁淤积症合并胃扭转,按照淤积程度运用中药和早期手法按摩治疗,能显著降低婴儿CMV感染胆汁淤积症合并胃扭转病程中发生肺炎的概率和严重程度,有利于治疗和预后。Objective： To study the preventative effects of massage to gastric volvulus （GV） and herb therapy to infantile cholestasis on secondary infant pneumonia for cytomegalovirus （CMV） infection induced infantile cholestasis complicated with gastric volvulus. Methods： One-hundred and twenty CMV infection induced infantile cholestasis complicated with GV inpatients were divided into two groups by the degree of cholestasis： normal syndrome （60 cases） and severe syndrome （60 cases）. Then each group was farther divided randomly into two subunits： basic treatment （30 cases） and massage treatment （30 cases）. Physical signs and biochemical indicators were assessed in all groups at the time of hospital admission and after procedure. Clinical curative effect of CMV infection induced infantile cholestasis, GV and pneumonia as well as occurrence of pneumonia, including severe pneumonia, were estimated after procedure in all groups. Results： After procedure, treatment groups showed a higher clinical curative effect on CMV infection induced infantile cholestasis, GV and pneumonia （P〈0.05, P〈0.01） and a significant lower incidence rate of pneumonia and severe pneumonia （P〈0.05, P〈0.01）. Conclusion： Timely massage treatment and herb therapy in grades could prevent occurrence of secondary infant pneumonia for CMV infection induced infantile cholestasis complicated with GV and also attenuate prognosis of CMV infection induced infantile cholestasis.
