摘 要:目的:通过观察针刺捻转补泻对自发性高血压大鼠(SHR)肾组织血管内皮生长因子(VEGF)表达的影响,探讨针刺捻转补泻手法对SHR肾脏保护的可能机制。方法:雄性SHR大鼠60只随机分为模型组、留针组、捻转补法组、捻转泻法组和平补平泻组,每组12只。12只WKY大鼠作为空白组。各针刺组分别采用不同的手法针刺"太冲穴",模型组和空白组只做与针刺组相同的抓捉刺激,不做针刺处理。28d后采用免疫组织化学的方法检测各组大鼠肾组织中VEGF的表达。结果:与空白组比较,模型组VEGF表达量增多,有显著性差异(P<0.01)。各针刺组VEGF表达量较模型组减少,有显著性差异(P<0.05,P<0.01)。捻转泻法组VEGF的表达较其他针刺各组显著减少(P<0.01)。结论:捻转泻法能有效地降低SHR血压水平,从而减轻因高血压所引起的肾脏损伤;捻转泻法能直接或间接通过降低SHR血压水平减少肾组织VEGF的表达;捻转补法与泻法存在效应的差异。Objective: To investigate the protection mechanism of reinforcing-reducing method by twirling needle on kidney of rats with spotaneously hypertensive (SHR) through observing the influence of reinforcing-reducing method by twirling needle on vascular endothelial growth factor (VEGF) expression in kidney tissue of SHR rats. Methods: 60 SHR rats were randomly divided into model group (group A), needle retaining group (group B), reducing method by twirling needle group (group C), reinforcing method by twirling needle group (group D) and even reinforcing-reducing method group (group E), and with 12 rats in each group. 12 WKY rats were served as control group (group F). Rats in acupuncture groups were acupunctured at Taicbong (LR 3) with corresponding needle manipulation technique, and rats in model and control groups were simulated with grasp as acupuncture groups. After treating for 28 days, the VEGF expression in kidney tissues of rats in each group was tested by using immunohistochemical method. Results: Compared with group F, the VEGF expression in kidney tissues of rats in group A was increased significantly (P〈0.01). The VEGF expression in kidney tissues of rats in group C was decreased significantly compared with the other acupuncture groups (P〈0.05, P〈0.01), and the difference in VEGF expression between group C and other treatment groups was significant (P〈0.01). Conclusion: Reducing method by twirling needle could decrease the blood pressure levels of SHR effectively to relieve the kidney injury caused by high blood pressure. Reducing method by twirling needle could decrease the VEGF expression in kidney tissues through decreasing blood pressure levels of SHR rats directly or indirectly. There is significant difference in effects between reducing method by twirling needle and reinforcing method by twirling needle.
