基于大肠癌早期筛查的中医体质类型分布规律研究

摘　　要：目的:通过调查浙江省海宁市8个社区40-74岁一般人群中医体质流行病学,描述一般人群及大肠癌高危人群中医体质类型分布规律。方法:采用《中医体质分类与判定自测表(中华中医药学会标准)》调查浙江省海宁市8个社区的40-74岁1 945名一般人群中医体质,并结合该人群大肠癌早期筛查结果分析一般人群及高危人群中医体质类型分布规律。结果:一般人群以平和质(67.46%)为主,其次是阳虚质(15.37%),血瘀质(5.35%)位列第三;高危人群中FOBT检查第一次血便阳性中,未见其他显著体质阳性率最高(12.12%),平和质最低(0.61%);FOBT检查第二次血便阳性项中,气虚质阳性率最高(8.11%),平和质最低(0.53%);具有慢性便秘、黏液血便、慢性腹泻、慢性阑尾炎、精神刺激史、慢性胆道疾病两项及两项以上者项中,血瘀质占所有调查人数血瘀质阳性率最高(5.77%),平和质最低(0.46%);本人有肠息肉史项中,气虚质最高(6.76%),平和质最低(0.23%);一级亲属有结直肠癌病史项中,血瘀质阳性率最高(2.88%)。结论:本次调查一般人群中约2/3的人群为平和体质,约1/3的人群为偏颇体质;偏颇体质中阳虚质较多见;高危人群中平和质人群显著偏低,阳虚质、血瘀质、气虚质显著偏高。Objective： Based on the epidemiological survey for TCM consti^otion of general population aged 40-74 years old in 8 communities in Halning city of Zhejiang province, to describe the distribution characters of TCM constitution types of general population and high risk population of colorectal cancer. Methods： According to the ＇self-test table of Chinese constitution classification and decision （Chinese medicine standard）＇, to investigate the TCM constitution of 1945 general population aged of 40-74 years old in 8 communities in Haining city of Zhejiang province, and analyze the distribution regularities of TCM constitution types of general population and high risk population of colorectal cancer based on the results of early colorectal cancer screening for the two population. Results： TCM constitution types of general people were given priority to gentler constitution （67.46%）, and followed by yang deficiency constitution （15.37%） and stagnant blood constitution （5.35%）. The results of the first fecal occult blood test （FOBT） for high risk people showed that the positive rate of constitution with no significant features was the highest （12.12%）, and the positive rate of gentler constitution was the lowest （0.61%）. The results of the second FOBT showed that the positive rate of qi deficiency constitution was the highest （8.11%）, and the positive rate of gentler constitution was the lowest （0.53%）. In the patients with two or more diseases as chronic constipation, mucobloody stool, chronic diarrhea, chronic appendicitis, mental stimulation history and chronic bile duct disease, the positive rate of stagnant blood constitution was the highest （5.77%）, and the positive rate of gentler constitution was the lowest （0.46%）. The positive rate of qi deficiency constitution was the highest in patients with colon polyp history （6.76%）, and the positive rate of gentler constitution was the lowest （0.23%）, while the positive rate of stagnant blood constitution was t
