通腑清降法对骨创伤合并感染患者SIRS/CARS平衡调控作用的研究

摘　　要：目的:观察通腑清降法组方的通腑净化汤对骨创伤合并感染后SIRS/CARS失衡状态的调控作用。方法:300例患者随机平均分入中药组和对照组,中药组采用通腑净化汤和西医常规治疗,对照组仅采用西医常规治疗,观察治疗前和治疗后第3、7天外周血炎性因子和T细胞亚群的变化,以及治疗后7天患者临床症状改善情况。结果:治疗后7天,中药组患者临床症状改善明显,总疗效明显优于对照组(P<0.05);治疗后3天、7天,与对照组比较,治疗组TNF-α、IL-6、IL-10水平明显降低(P<0.05),CD+4以及CD+4/CD+8比值升高。结论:通腑净化汤联合西医常规治疗骨创伤合并感染后SIRS/CARS失衡状态,能有效减少炎症介质产生,抑制炎症反应,调节免疫功能,调控SIRS/CARS平衡。Objective： To observe the regulation effects of Tongfu Jinghua Tang（ 通腑净化汤） on SIRS /CARS balance in infection patients after bone trauma. Methods： 300 patients were randomly divided into the treatment group and control group. Traditional Chinese medicine group with Tongfu Jinghua Tang and conventional western medicine treatment,the control group only with conventional western medicine treatment,inflammatory factors and regulatory T cells level changes were observed before and after treatment 3 days and 7 days,and the clinical symptoms of the patients after treatment 7 days were observed. Results： After treatment 7days,the clinical symptoms of treatment group improved significantly,the total effect was better than that of control group（ P〈0. 05）; compared with the control group,TNF- α,IL- 6,IL- 10 levels decreased significantly in the treatment group after treatment 3 days and 7 days（ P〈0. 05）,CD4^＋and CD4^＋/ CD8^＋were increased. Conclusion： Tongfu Jinghua Tang combined with routine western medicine can effectively reduce the inflammatory mediators,inhibit inflammation,regulating immune function,regulating SIRS / CARS balance in the infection after bone trauma.
