日间手术集中式与分散式管理模式的研究及实践

摘　　要：目的:探讨日间手术集中式与分散式管理模式之间的差异。方法:通过回顾性分析上海交通大学医学院附属仁济医院近期开展日间手术的实践,选择2013年6月1日-2014年9月30日之间集中式和分散式不同管理模式下同期开展腹腔镜下胆囊切除术和良性甲状腺疾病(包括甲状腺良性肿瘤、结节性甲状腺肿、甲状腺结节),对日间手术的平均住院日和人均医疗总费用进行比较。结果:集中式管理模式下日间手术患者平均住院日和住院总费用均低于分散式管理的同类患者。结论:日间手术集中式管理模式能更好地缩短平均住院日和控制住院总费用。Objectives：To explore the difference of centralized mode and distributed mode in the management of day surgery. Methods：By reviewing the practice of day surgery in Renji Hospital from June 1, 2013 to September 30, 2014, the average length of stay, average discharge expenditure in diseases of＂Laparoscopic cholecystectomy and benign thyroid diseases （including thyroid benign tumor, nodular goiter, thyroid nodules ）between day surgery of centralized mode and distributed mode were compared. Results：It is shown that average length of stay is shortened, average discharge expenditure is reduced in the day surgery of centralized mode. Conclusions：Day surgery of centralized mode could shorten average length of stay and reduce average discharge expenditure.
