宣白承气汤治疗机械通气急性肺损伤临床研究

摘　　要：目的:观察鼻饲宣白承气汤对给予机械通气急性肺损伤患者呼吸力学指标的影响。方法:118例急性肺损伤机械通气患者随机平均分为对照组和研究组。对照组给予常规药物治疗,研究组在对照组基础上鼻饲宣白承气汤。记录并分析两组患者气道峰压、平台压、呼气末正压、吸氧浓度通气指标、机械通气时间、入住重症加强护理病房时间以及生活质量情况。结果:研究组气道峰压、平台压、呼气末正压和吸氧浓度通气指标值显著低于对照组(P<0.05);研究组机械通气时间和入住重症加强护理病房时间明显短于对照组,生活质量评分比对照组高,差异均有统计学意义(P<0.05)。结论:鼻饲宣白承气汤治疗急性肺损伤机械通气临床疗效较好,可显著改善患者的呼吸力学指标。Objective ：To observe the breathing mechanics indicative influence on the acute lung injury with mechanical ventilation by Xuanbai Chengqi Tang. Methods：118 cases were randomly divided into control group and treatment group. The control group was given convention drug therapy,while the treatment group was given Xuanbai Chengqi Tang by nasal feeding based on the control group. Re- cording and analyzing the two groups of patients with airway peak pressure, platform, breathe out at the end of the positive pressure, oxy- gen concentration in the ventilation index, mechanical ventilation time and ICU admission time and quality of life. Results ： In the treat- ment group, airway peak pressure, platform, positive pressure and oxygen concentration at the end of the ventilation index was signifi- candy lower than the control group（ P 〈 0.05 ） , mechanical ventilation time and ICU admission time significantly shorter than the control group, the quality of life score higher than the control group,. The difference had statistical significance （P 〈 0.05 ）. Conclusion：Xuan- bai Chengqi Tang by nasal feeding on the treatment of acute lung injury with Mechanical ventilation has good clinical curative effect, which can significantly improve the patient＇s respiratory mechanics index.
