冠心病血瘀证与凝血功能及血小板参数相关性分析

摘　　要：目的探讨冠心病血瘀证与凝血功能及血小板参数的相关性。方法选择74例冠心病患者,其中血瘀证患者50例,非血瘀证患者24例。检测两组患者凝血酶原时间(PT)、部分活化凝血活酶时间(APTT)、纤维蛋白原(FIB)、血小板计数(PLT)、血小板分布宽度(PDW)、平均血小板体积(MPV)、大型血小板比率(PLCR)以及血小板比容(PCT),计算国际标准化比值(INR),比较两组各指标水平,并采用Pearson相关性检验分析冠心病血瘀证与各指标的相关性,采用二元Logistic回归分析探讨冠心病血瘀证的影响因素。结果血瘀证组PT、INR明显小于非血瘀证组(P<0.05)。两组血小板各项指标比较,差异无统计学意义(P>0.05)。冠心病血瘀证与PT、INR有相关性(P<0.05)。INR是冠心病血瘀证的影响因素。结论冠心病血瘀证与凝血功能关系密切,表现在PT缩短,INR降低。Objective To investigate the correlation of blood stasis syndrome of coronary heart disease with coagulation function and platelet parameters. Methods 74 patients with coronary heart disease were enrolled, including 50 cases with blood stasis syndrome and the other 24 without it. Prothrombin time （ PT）, activated partial thromboplastin time （ APTT）, fibrinogen （ FIB ）, platelet count （ PLT）, platelet distribution width （ PDW）, mean platelet volume （MPV） , platelet large cell ratio （PLCR） and plateletcytocrit （PCT） in both groups were detected. International normalized ratio （INR） was calculated. All the parameters were compared between two groups. Pearson correlation analysis was used to evaluate the correlation of blood stasis syndrome with parameters. Binary Logistic regression analysis was used to investigate the influential factors of blood stasis syndrome of coronary heart disease. Results PT and INR in blood stasis syndrome group were significantly lower than those in non-blood stasis syndrome group （P 〈 0. 05 ）. Platelet parameters showed no significant difference between two groups （P 〉 0. 05 ）. Blood stasis syndrome of coronary heart disease was correlated with PT and INR （P 〈 0.05 ）. INR is an influential factor of blood stasis syndrome of coronary heart disease. Conclusion There is close relationship between blood stasis of coronary heart disease and coagulation function, reflected by lower PT and INR.
