复方大黄煎剂联合紫草油治疗Ⅲ、Ⅳ期压疮30例临床观察

摘　　要：目的观察中药复方大黄煎剂联合紫草油治疗Ⅲ、Ⅵ级压疮的临床疗效。方法将60例Ⅲ、Ⅳ级压疮患者随机分为治疗组和对照组30例。治疗组使用中药复方大黄煎剂冲洗湿敷患处,待痂皮脱落后外敷紫草药油,每2-3天换药1次。对照组创面常规消毒后喷洒重组人碱性成纤维细胞生长因子,每天换药1次。于治疗后10、20、30天比较两组患者的临床疗效,并观察患者创面腐肉脱落时间、新生肉芽出现时间及创面愈合时间。结果两组随着治疗时间的延长,治愈率和总有效率均明显提高（P〈0.05）。两组治疗后10天组间比较治愈率和总有效率差异无统计学意义（P〉0.05）,治疗组治疗后20天、30天治愈率和总有效率均高于对照组（P〈0.05）。治疗组腐肉脱落时间、新生肉芽出现时间、创面愈合时间均短于对照组（P〈0.05）。结论复方大黄煎剂联合紫草油治疗Ⅲ、Ⅳ级压疮疗效优于碱性成纤维细胞生长因子。Objective To observe the clinical efficacy of Compound Rhubarb decoction combined with Lithospermum oil in the treatment of III and VI phase of pressure ulcers. Methods Totally 60 patients with III and VI phase of pressure ulcers were randomly divided into treatment group and control group, with 30 cases in each group. In the treatment group, the wound was washed and wet applied with Compound Rhubarb decoction. After the crusts fell off, the wound was applied with lithospermum oil, and dressed every 2 or 3 days. In the control group, after conventional disinfection of the wound, the recombinant human basic fibroblast growth factor was sprayed once daily. After treatment of 10, 20 and 30 days, clinical curative effect in the two groups were compared, and the patients＇ wound carrion shedding time, granulation appeared time and wound healing time were observed. Results The cure rate and total effective rate were significantly improved in both groups with the extension of treatment time （ P 〈 0.05 ）. After 10 days of treatment, there were no statistically significant differences in the cure rate and total effective rate between the two groups （ P 〉 0. 05 ）. After 20 and 30 days of treatment, the cure rate and total effective rate in the treatment group were higher than those in the control group （P 〈 0. 05 ）. The carrion shedding time, granulation time, and wound healing time in the treatment group were shorter than those of the control group （ P 〈 0. 05 ）. Conclusion Compound Rhubarb decoction combined with lithospermum oil has better curative effect in treatment of III and VI phase of pressure ulcers than basic fibroblast growth factor.
