基于德尔菲法的寻常型银屑病常见证候诊断指标研究

摘　　要：目的通过专家问卷调查建立寻常型银屑病常见证候诊断指标体系。方法制定寻常型银屑病常见证候即血热证、血瘀证、血燥证诊断指标体系专家咨询问卷,遴选全国范围内皮肤病领域从事中医、中西医结合临床工作的专业人员32位,分别进行两轮问卷调查。评价指标采用五尺度评分法,并采用百分权重法对指标进行赋权;统计专家积极系数及权威程度,并采用算术均数、等级和、满分比分析专家评价意见集中程度,采用变异系数、协调系数分析专家评价意见协调程度。结果两轮问卷专家积极系数分别为81.3%、96.9%,权威程度分别为0.90、0.96。第一轮问卷变异系数波动在0.06-0.43之间,第二轮问卷变异系数波动在0.05-0.32之间;第一轮问卷各证型协调系数分别为血热证0.375、血瘀证0.488、血燥证0.379,第二轮问卷各证型协调系数分别为血热证0.276、血瘀证0.357、血燥证0.256。根据各证候诊断指标的权重系数及专家意见集中程度、协调程度决定各指标对证候诊断的贡献度,最终血热证有15个诊断指标,血瘀证有10个诊断指标,血燥证有13个诊断指标。结论初步建立银屑病常见证候即血热证、血瘀证、血燥证诊断指标体系,可为本病证候诊断标准的确立提供参考。Objective To establish a system of diagnostic index for common symptoms in psoriasis vulgaris by expert questionnaire survey. Methods An expert consulting questionnaire was designed for diagnostic index system of common symptoms of psoriasis vulgaris including syndromes of blood heat, blood stasis and blood dryness. 32 experts engaging in dermatology of traditional Chinese medicine or integrative traditional Chinese and Western medicine were selected nationwide to response the survey. They answered the questionnaires for two rounds. Five rating scale was used for evaluating index and stratified analyzed method was applied for weighting. Experts＇ positive coefficients and authoritative degrees were calculated. Arithmetic mean, rank sum and full mark ratio were adopted to analyze the concentrative degree of experts＇ opinions. Harmonious degree of experts＇ opinion was evaluated by variable and coordination coefficient. Results Positive coefficients of experts＇ questionnaire in first and second rounds were 81.3% and 96.9%, respectively, whereas authoritative degrees were 0.90 and 0. 96. Variable coefficient of the first round questionnaire was 0.06 - 0.43 and that of the second round was 0.05 - 0. 32. Harmonious coefficients of different syndromes in the first round questionnaire were 0. 375,0. 488 and 0. 379 for syndromes of blood heat, blood stasis and blood dryness, respectively ; and that in the second round were 0. 276, 0. 357 and 0. 256, respectively. Contribution of each parameter for diagnosis was indicated according to weighting coefficients of diagnostic indexes of different syndromes and concentrative and harmonious degrees of experts＇ opinions. 15, 10 and 13 diagnostic indexes were determined eventually for syndromes of blood heat, blood stasis and blood dryness, respectively.
