观察β受体阻滞剂联合不同起始剂量胺碘酮治疗急性顽固性室性心动过速的效果及安全性

摘　　要：目的：观察β受体阻滞剂联合不同起始剂量胺碘酮治疗急性顽固性室性心动过速的效果及安全性。方法选择曲沃县中医医院2012年1月至2013年12月收治的80例急性顽固性室性心动过速患者为研究对象，按照入院先后顺序将其分为两组，每组40例，均行β受体阻滞剂＋胺碘酮治疗，A 组胺碘酮起始剂量为3 mg/ kg，B组为1.5 mg/ kg，对两组临床疗效、不良反应、治疗前后心电图情况进行比较。结果 A 组治疗总有效率、不良反应发生率分别为95%、10%，与 B 组比较差异未见统计学意义（P ﹥0.05）。两组治疗后心率、QTc 间期等心电图指标比较差异未见统计学意义（P ﹥0.05）。结论相比1.5 mg/ kg 胺碘酮用药，3 mg/ kg 胺碘酮治疗急性顽固性室性心动过速疗效更好，不良反应相对多，但两者比较差异未见统计学意义。Objective To observe the efficacy and safety of β-receptor blockers in combination with different initial dose amiodarone in the treatment of acute refractory ventricular tachycardia. Methods Eighty cases of acute refractory ventricular tachycardia from January 2012 to December 2013 were chose as the research objects,according to admission order were divided into two groups,each group with 40 cases,both were treated with β-receptor blockers ＋ amiodarone,patients in group A were treated with amiodarone starting dose of 3 mg/ kg,patients in group B were treated with amiodarone starting dose of 1. 5 mg/ kg,the clinical curative effect,adverse reaction and the situation of electrocardiogram（ecg）before and after treatment was compared. Results Total effective rate,incidence of adverse reactions of group A was 95% ,10% ,compared with those of group B（90% ,2. 5% ）,there was no significant difference（P ﹥ 0. 05）. In addition,of the two groups after treatment,the heart rate,the QTc interphase ecg indexes such comparison had no significant difference（P ﹥0. 05）. Conclusions Compared to 1. 5 mg/ kg amiodarone,3 mg/ kg of amiodarone in treatment of acute refractory ventricular tachycardia has better curative effect,the adverse reaction is relatively more,but there is no significant difference.
