维持性血液透析患者静息心率与心脏结构及功能相关指标关系研究

摘　　要：目的探讨维持性血液透析患者静息心率(RHR)水平与心脏结构及功能相关指标的关系。方法选取2012年5月至2014年5月该院血液净化中心维持性血液透析患者59例作为研究对象,根据RHR水平将其分为A组(RHR<70次/分,18例)、B组(70次/分≤RHR<80次/分,20例)和C组(RHR≥80次/分,21例)。比较各组左心室舒张末期内径(LVEDd)、左心室收缩末期内径(LVESd)、室间隔厚度(IVST)、左心室射血分数(LVEF)、舒张早期和舒张晚期二尖瓣口最大血流速度之比(E/A)和血清氨基末端脑利钠肽前体(NT-pro BNP)水平。结果 RHR水平与LVEF、E/A呈负相关(r=-0.47、-0.52,P<0.05),与NT-pro BNP水平、LVEDd、LVESd、IVST呈正相关(r=0.60、0.71、0.69、0.53,P<0.05)。结论维持性血液透析患者RHR增快与其心功能下降密切相关,RHR水平可在一定程度上反映患者心脏功能和结构异常情况。Objective To investigate the relationship between the resting heart rate（RHR） levels with the cardiac structure and function indexes in the patients with maintenance hemodialysis. Methods Fifty-nine cases of maintenance hemodialysis in the blood purification center of our hospital from May 2012 to May 2014 were selected as the research subjects and divided into the group A（RHR70 beats/min,18 cases）,B（70 beats/min ≤RHR80 beats/min,20 cases） and C（RHR≥80 beats/min,20 cases）according to the RHR levels. The left ventricular end-diastolic diameter（LVEDd）,left ventricular end-systolic diameter（LVESd）,interventricular septum thickness（IVST）,left ventricular ejection fraction（LVEF）,early diastolic and late diastolic mitral flow velocity ratio（E/A） and serum N-terminal B-type natriuretic peptide（NT-pro BNP） level were compared among 3 groups. Results The RHR level was negatively correlated with LVEF and E/A（r=-0.47、-0.52,P0.05）,and positively correlated with the NT-pro BNP level,LVEDd,LVESd and IVST（r=0.60、0.71、0.69、0.53,P0.05）. Conclusion The increase of RHR is closely correlated with the cardiac function decrease in the patients with maintenance hemodialysis,so the RHR level can reflect the abnormal situation of the cardiac function and structure in some extent.
