清眩汤治疗后循环缺血性眩晕113例临床观察

摘　　要：目的观察清眩汤治疗后循环缺血性眩晕的临床疗效。方法将220例后循环缺血性眩晕患者随机分为2组,治疗组113例予清眩汤治疗,对照组107例予盐酸氟桂利嗪胶囊治疗,2组均14 d为1个疗程,治疗1个疗程。比较2组临床疗效、中医证候疗效及2组治疗前后中医证候积分变化;经颅多普勒(TCD)检测2组治疗前后椎—基底动脉平均血流速度;检测2组治疗前后血液流变学指标变化;检测2组治疗前后血脂指标变化[总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)]。结果治疗组治疗后中医证候积分均低于本组治疗前、对照组治疗后(P<0.05)。治疗组治疗后椎—基底动脉平均血流速度均较本组治疗前提高(P<0.05,P<0.01),其中右椎动脉血流速度较对照组治疗后提高(P<0.05)。治疗组治疗后血液流变学指标均较本组治疗前降低(P<0.05),其中全血黏度(低切)低于对照组治疗后(P<0.05)。治疗组治疗后血脂指标除HDL-C外均较本组治疗前改善(P<0.05),且较对照组治疗后改善明显(P<0.05)。结论清眩汤治疗后循环缺血性眩晕疗效显著,且安全、无毒副作用,说明从脾胃辨治后循环缺血性眩晕合理有效。Objective To observe the clinical effects of Qingxuan decoction on posterior circulation ische-mia vertigo （ PCIV） .Methods 220 PCIV patients were randomly divided into two groups.113 patients in treatment group were treated by Qingxuan decoction.107 patients in control group were treated by flunarizine hydrochloride cap-sules.The course was 14 days.The curative effects, Traditional Chinese medicine （ TCM） symptom effects and the changes of TCM symptom score before and after one course were observed in two groups.The vertebra-basilar artery blood velocity were detected by transcranial doppler （ TCD） in two groups, and the hemorheology indexes were meas-ured by blood viscometer, the changes of total cholesterol （ TC） , triglyceride （ TG） , high density lipoprotein choles-terol （ HDL-C） and low density lipoprotein cholesterol （ LDL-C） before and after treatment were detected .Results The TCM symptom score after treatment was decreased as compared with that in before treatment, as well as in control group （P〈0.05）.The vertebra-basilar artery blood velocity after treatment was increased in treatment group （P〈0.05,P〈0.01）, and the right vertebral artery blood velocity after treatment was increased as compared with that in control group （P〈0.05）.The hemorheology indexes after treatment was decreased in treatment group （P〈0.05）, and the whole blood viscosity （low cut） after treatment was lower than that in control group （P〈0.05）.The serum lipid parameters after treatment were improved in treatment group except HDL-C （P〈0.05）, and the im-provement was more obvious than that in control group （P〈0.05）.Conclusion Qingxuan decoction has definite clinical effects on PCIV, safety and without toxicity and side effects, show that treatment from spleen and stomach on PCIV is reasonable and valid.
