摘  要:目的探讨2型糖尿病患者血清总胆红素与糖尿病视网膜病变之间的关系。方法回顾性分析2014年2月至2014年8月于本院住院治疗的2型糖尿病患者237例。对合并与未合并糖尿病视网膜病变两组患者的临床特征进行分析比较,并对糖尿病视网膜病变的危险因素进行多因素Logistic回归分析。结果 (1)237例入选患者中,合并糖尿病视网膜病变的患者共80例(占33.6%)。(2)与未合并糖尿病视网膜病变组相比,合并糖尿病视网膜病变组患者的年龄更大(P<0.05),糖尿病病程更久(P<0.01),空腹血糖较对照组高(P<0.01),尿微量白蛋白/尿肌酐较对照组升高(P<0.01),但是总胆红素水平较对照组显著下降(P<0.01)。(3)Logistic回归分析显示:糖尿病病程、尿微量白蛋白/尿肌酐及总胆红素水平与糖尿病视网膜病变之间的关系有统计学意义。结论本研究结果显示糖尿病视网膜病变的发生率与血清胆红素水平的下降有一定的相关性,提示血清总胆红素可能是糖尿病视网膜病变的一种有益的标记物。Objective To evaluate the relationship between total bilirubin and diabetic retinopathy. Methods A total of 237 patients with T2DM who admitted to the Department of Endocrinology of Beijing Haldian Hospital t^om February 2014 to August 2014 were reviewed. Results Of the 237 patients, the prevalence of diabetic retinopathy (DR) was 80 cases (33.76%) according to clinical ophthalmic examination. Compared with those NDR patients, the age of patients with DR was significantly higher (P〈0.05) and duration of DM was significantly longer (P〈0.01), in addition, FBG of the DR group was significantly higher (P〈0.01). Microal albuminuria was higher, but total bilirubin was significantly lower (P〈0.01). The duration of DM (P--0.003), urine albumin-ereatinine ratio (/=-0.028) and total bilirubin level (P=-0.001) in multiple Logistic regression analysis were statistically significantly associated with the incidence of diabetic retinopathy. Conclusion The present study indicates that total bilirubin may be useful biornarkers for increased risk of diabetic retinopathy since retinopathy in patients with T2DM was linked to decrease serum total bilirubin level.
