摘 要:目的观察冷缺血期氢气膨肺对移植肺脏氧化应激和细胞凋亡的影响。方法冷缺血期,对照组(control组)采用40%氧气+60%氮气膨肺,氢气组(H2组)采用3%氢气+40%氧气+57%氮气膨肺,20min置换一次肺内气体,180 min后肺移植。受体于移植前、再灌注3 min、60 min、120 min、180 min行动脉血气分析,再灌注180 min时检测移植肺组织丙二醛(MDA)浓度和超氧化物歧化酶(SOD)活性,观察细胞凋亡及caspase-3蛋白表达情况,并行肺组织损伤评分(LIS)。结果与control组相比,再灌注120 min、180 min H2组氧和指数(Pa O2/Fi O2)、BE值、p H值显著增加(P<0.05)。H2组MDA浓度、LIS、caspase-3蛋白表达和细胞凋亡显著低于control组,SOD活性显著增加(P<0.05)。结论冷缺血期氢气膨肺能够抑制移植肺脏氧化应激损伤和细胞凋亡。Objective To observe the effect of lung inflation with hydrogen during cold isehemia phase on oxidative stress and cell apoptosis of lung grafts in rats. Methods During cold ischemia phase, the control group was given the inhalation of 40% oxygen and 60% nitrogen, and the H2 group was given 3% hydrogen, 40% oxygen and 57% nitrogen. The mixed gases were replaced every 20 min and both were kept at 4~C for 180 min. Arterial blood gas was analyzed in recipients before transplantation, and 3 min, 60 min, 120 min, 180 min after reperfusion. Recipients were sacrificed 180 min after reperfusion to detect the malonaldehyde (MDA) content and superoxide dis- mutase (SOD) activity. Cell apoptosis, caspase-3 protein expression and lung injury score (LIS) of lung grafts were also detected. Results The partial pressure of arterial oxygen (PaO/)/fraction of inspired oxygen ( FiO2 ), base excess (BE) , and pH value increased more obviously in the H2 group than in the control group 120 rain and 180 min after reperfusion ( P 〈 0.05 ). The MDA content, LIS, cell apoptosis and caspase-3 protein expression in lung grafts decreased and SOD activity increased more significantly in the H2 group than in the control group ( P 〈 0. 05 ). Conelusion Lung inflation with hydrogen during cold ischemia phase can inhibit oxidative stress and cell apoptosis in lung grafts in rats.
