摘  要:目的:观察布地奈德氧气驱动雾化吸入对新生儿呼吸窘迫综合征(NRDS)呼吸困难症状改善时间及吸氧时间的影响并评价疗效。方法:选取2011年6月至2013年6月在我院产科出生的NRDS早产儿98例,随机分为观察组和对照组各49例。对照组给予营养支持、限制液体摄入量、氧疗、使用外源性肺表面活性物质(PS)、控制感染等综合治疗,观察组在综合治疗基础上加用布地奈德混悬液0.5 mg加生理盐水1 m L氧气驱动雾化吸入10 min,每12 h一次,治疗5 d后评价疗效。结果:观察组总有效率87.8%,高于对照组的71.4%(P<0.05);观察组三凹征消失时间、呼吸频率恢复正常时间、吸氧时间均短于对照组(P<0.05)。结论:应用布地奈德氧气驱动雾化吸入辅助治疗NRDS早产儿能更快缓解呼吸困难,缩短用氧时间,提高临床疗效,值得临床推广应用。Objective: To investigate the clinical effect of the budesonide atomized inhalation for neonatal respiratory distress syndrome in time of breathing difficulties improvement and oxygen inhalation. Methods: Ninety-eighty premature who were diagnosed as neonatal respiratory distress syndrome from June 2011 to June 2013 were randomly divided into an observation group (n = 49 ) and a control group (n =49). The control group was given comprehensive treatment including poractant alfa injection, nutrition support, fluid restriction, oxygen therapy. The observation group was given budesonide O. 5 mg with normal saline 1 mL inhalation by oxygen compression pump, 10 min per time, twice a day. The clinical efficacy was observed in two groups after 5 days. Results: The total effective rate of the control group and observation group were 71. 4% and 87. 8% respectively, there was significant difference between two groups ( P〈0.05 ) , and there were also significant differences of three depressions sign disappear time, respiratory frequency back to normal time, oxygen inhalation time between two groups ( P 〈 0.05 ). Conclusion: The combination of budesonide can relieve breathing difficulties, cut down the time of oxygen inhalation and improve the effect in premature with respiratory distress syndrome, is worthy of clinical popularization and application.
