降钙素原与感染细胞监测在呼吸机相关性肺炎的早期诊断价值

摘　　要：目的探讨降钙素原(PCT)与感染细胞百分比(PIC)在呼吸机相关性肺炎(VAP)的早期诊断中的参考价值,以指导早期对呼吸机相关性肺炎做出及时的诊断和治疗。方法选取于2010年6月-2012年6月于医院接受机械通气>48h且怀疑为呼吸机相关性肺炎的60例患者作为研究对象,按照诊断标准分为观察组32例与对照组28例,对两组患者的PCT水平和每百个吞噬细胞内PIC进行对比,数据采用SPSS 15.0软件进行统计分析。结果对照组患者的PCT水平和每百个吞噬细胞内PIC分别为(1.57±0.80)ng/ml和(1.17±0.31)%,观察组患者的PCT水平和PIC分别为(3.19±1.39)ng/ml和(3.03±1.43)%,两组数据差异有统计学意义(P<0.01)。结论降钙素原与PIC在呼吸机相关性肺炎的早期诊断和治疗具有参考价值。OBJECTIVE To explore the value of procalcitonin （PCT） and percentage of infected cells （PIC） in early diagnosis of ventilator‐associated pneumonia （VAP） so as to provide guidance for early diagnosis and treatment of VAP .METHODS A total of 60 patients with suspected VAP who underwent mechanical ventilation for more than 48 hours from Jun 2010 to Jun 2012 were recruited as the study objects and were divided into the observation group with 32 cases and the control group with 28 cases according to the diagnostic criteria .The level of PCT and the PIC in each one hundred phagocytes were observed and compared between the two groups of patients ,and the sta‐tistical analysis was performed with the use of SPSS 15 .0 software .RESULTS The level of PCT of the control group was （1 .57 ＆#177; 0 .80）ng/ml ,the observation group （3 .19± 1 .39）ng/ml;the PIC in each one hundred phago‐cytes of the control group was （1 .17 ± 0 .31）% ,the observation group（3 .03 ± 1 .43）% ,and there was significant difference （P〈0 .01） .CONCLUSION PCT and PIC may have reference value in early diagnosis and treatment of VAP .
