急性白血病患者医院感染直接经济损失调查与分析

摘　　要：目的：探讨急性白血病患者医院感染所致的直接经济损失，为制定感染控制政策与措施提供参考依据。方法对2011年1月－2013年12月入住医院某血液病房的362例急性白血病患者进行回顾性调查，采用1∶1方法分为感染组与非感染组，各181例，比较两组患者的住院费用和住院天数差异，数据采用SPSS 19．0软件进行统计分析。结果感染组患者住院平均总费用为34394元，非感染组患者为14049元；不同年龄段患者住院平均总费用差值不同，在13969～27179元，比值为2．20～2．56；不同年龄段患者的平均住院天数在8～11．5 d；组间比较，差异均有统计学意义（P＜0．05）。结论医院感染发生后加重了患者的经济负担，并且直接经济损失因患者年龄段不同而有所差异。OBJECTIVE To investigate the direct economic losses caused by nosocomial infections on acute leukemia patients so as to supply scientific basis for formulate relevant policies and measures to control infections . METHODS Totally 362 acute leukemia patients in blood center in a tertiary teaching hospital from Jan .2011 to Dec .2013 were selected as study objects .Their clinical data were investigated retrospectively .By the means of a 1∶1 ratio of matching control ,they were divided into infected group and non‐infected group with 181 cases each . The hospitalization cost and time were compared between the two groups .All data were statistically analyzed by SPSS 19 .0 software .RESULTS The average total expenses for hospitalization in the infected group were 34 ,394 yuan and 14 ,049 yuan in the non‐infected group .The average hospitalization expenses in different age groups were various too ,from 13 ,969 to 27 ,179 yuan and the ratios were from 2 .20 to 2 .56 .The average hospital stays were 8 to 11 .5 d in different age groups .The comparison between the two groups were significant （P〈0 .05） . CONCLUSION After the occurrence of nosocomial infections ,there will be an increase in hospitalization costs and prolonged hospital time in acute leukemia patients ,which were affected by age .
