逐瘀通脉饮治疗中晚期下肢动脉硬化闭塞症介入术后疗效观察

摘　　要：目的观察逐瘀通脉饮在中晚期下肢动脉硬化闭塞症（ASO）介入术后的应用效果。方法将61例中晚期下肢ASO患者随机分为2组。对照组30例予下肢ASO介入治疗，术后常规予以抗感染、抗凝治疗，治疗组31例在对照组治疗基础上加用中药逐瘀通脉饮口服治疗。2组均治疗6个月后观察疗效，并随访12～18个月。比较2组治疗前后踝肱指数（ABI）、经皮氧分压、跛行距离、血液流变学指标[全血黏度（高切）、全血黏度（低切）、红细胞比容、血浆黏度]、血脂指标[甘油三酯（TG）、总胆固醇（TC）、高密度脂蛋白胆固醇（HDL-C）、低密度脂蛋白胆固醇（LDL-C）]水平变化及远期再狭窄情况。结果2组治疗后ABI、经皮氧分压、跛行距离较本组治疗前均改善（P〈0．05），且治疗组改善情况优于对照组（P〈0．05）；2组治疗后全血黏度（高切）、全血黏度（低切）、红细胞比客及血浆黏度较本组治疗前均改善（P〈0．05），且治疗组改善情况优于对照组（P〈0．05）；2组治疗后TC、TG、HDL-C及LDL-C较本组治疗前均改善（P〈0．05），且治疗组改善情况优于对照组（P〈0．05）；治疗纽远期再狭窄复发率6．45％，低于对照组（16．67％，P〈0．05）。结论逐瘀通脉饮可以明显改善中晚期下肢ASO介入术后患者的临床症状、体征，改善血液流变学和血脂指标，降低远期再狭窄复发率，安全可靠，具有良好的临床应用前景。Objective To observe the effects of Zhuyu - tongmai drink on lower extremity arteriosclerosis obliterans （ASO） in middle and advanced stage after intervention operation. Methods 61 lower extremity ASO pa- tients were randomly divided into two groups. 31 patients in control group received anti - infective therapy after ASO intervention operation of lower extremity. 31 patients in treatment group were treated by Zhuyu - tongmai drink on the basis of control group treatment. The curative effect was observed after six months with follow - up of 12 to 18 months. The ankle- braehial index （ABI） , transcutaneous oxygen pressure, claudication distance, whole blood viscosity （high - cut）, whole blood viscosity （ low - cut）, hematoerit value, plasma viscosity, triglyceride （ TG ）, total cholesterol （TC）, high density lipoprotein cholesterol （ HDL - C ）, low density lipoprotein cholesterol （ LDL - C ） and the long - term restenosis condition before and after tireatment were compared between two groups. Results The ABI, transcuta-neous oxygen pressure and elaudication distance after treatment were improved in two groups （ P 〈 0.05 ） , and the improvement in treatment group was supenor to that in control group （ P 〈 0.05 ）. The whole blood viscosity （ high - cut） ,whole blood viscosity （low- cut）, hematocrit value, plasma viscosity after treatment were improved in two groups （ P 〈 0.05 ）, and the improvement in treatment group was superior to that in control group （ P 〈 0.05 ）. The TC, TG, HDL - C and LDL - C after treatment were improved in two groups （ P 〈 0.05 ）, and the improvement in treatment group was superior to that in control group （ P 〈 0.05 ）. The long - term restenosis rate of treatment group （6.45%） was lower than that in control group （ 16.67% ,P 〈 0. 05 ）.
