3种类型心肌病QRS时限对比及超声心动图相关性分析

摘　　要：目的观察扩张型心肌病(DCM)、肥厚型梗阻性心肌病(HCM)及高血压性心肌病(HC)3种类型心肌病QRS时限的差异,并探讨QRS时限与左室舒张末期内径(LVEDD)、左室收缩末期内径(LVESD)的相关性。方法选择DCM、HCM、HC患者各20例,测量3组患者QRS时限、LVEDD、LVESD,分析QRS时限的差异及各组QRS时限与LVEDD、LVESD的相关性。结果 3种类型心肌病QRS时限比较,DCM组QRS时限明显延长,方差分析3种类型心肌病QRS时限对数差异有统计学意义(P<0.05),DCM组QRS时限较HCM及HC组明显延长(P<0.05),HCM组与HC组QRS时限差异无统计学意义(P>0.05)。DCM组QRS时限与LVEDD、LVESD呈正相关(P<0.05),HCM及HC与LVEDD、LVESD无明显相关性。结论 3种类型心肌病QRS时限与彩色多普勒超声心动图相关性存在明显差别。可能原因为3种病变病情进展对心脏结构与传导系统的影响存在差异,体表十二导联常规心电图与彩色多普勒超声心动图的不同表现可为3种类型心肌病临床治疗提供参考。Objective To observe the difference of QFS duration on dilated cardiomyopathy （DCM）, Hy- pertrophic obstructive cardiomyopathy （HCM） and Hypertensive cardiomyopathy （ HC）, and to investigate the corre- lation of QRS duration, left ventricular end - diastolic diameter （LVEDD） and left ventricular end systolic diameter （LVESD）. Methods Patients with DCM, HCM and HC were selected into three groups respectively, 20 patients in each group. The QRS duration, LVEDD and LVESD were measured in three groups, and the difference of QRS dura- tion was analyzed, as well as the the correlation of QRS duration, LVEDD and LVESD. Results There was signifi- cant difference among three types of cardiomyopathy on ORS duration by the analysis of variance （ P 〈 0.05 ）, the QRS duration in DCM group was significantly prolonged as compared with that in HCM and HC group （ P 〈 0.05 ）, and there was no significant difference between HCM and HC group on QRS duration （ P 〉 0. 05 ）. The QRS duration in DCM group was positively associated with LVEDD and LVESD （P 〈 0.05 ）, and there was no obvious correlation in HCM and HC groups . Conclusion There was significant difference of the correlation of QRS duration and color Doppler ultrasound cardiogram among three types of cardiomyopathy. The possible cause may be the different effects of three types of cardiomyopathy on heart structure and conduction system. The different expressions of routine elec- trocardiogram and color Doppler ultrasound cardiogram can supply the referential basis for clinical treatment of three types of cardiomyopathy.
