温痹散热敷治疗强直性脊柱炎疗效观察

摘　　要：目的：观察和评价温痹散热敷治疗强直性脊柱炎的临床疗效。方法：将56例强直性脊柱炎患者随机分为试验组、对照组各28例。两组均口服柳氮磺吡啶片、沙利度胺片、塞来昔布胶囊,试验组同时运用自拟温痹散局部热敷。治疗4周后,评价夜间腰背痛和总体腰背痛VAS（0～10）评分、晨僵时间、腰部活动度（指地距离、Schober实验）、患者对全身总体病情VAS（0～10）评分,化验血沉、c-反应蛋白、血、尿常规、肝、肾功能等。结果：治疗4周后,与对照组比较,试验组减少腰背痛VAS（0～10）评分参数（P〈0.01）,缩短晨僵时间（P〈0.01）;改善血清ESR、CRP参数（P〈0.1）;但对指地距离、Schober实验、总体病情VAS（0～10）评分参数的影响不明显（P﹥0.05）。两组不良反应发生率差异无统计学意义（P〉0.05）。结论：温痹散热敷中西医结合治疗中医辨证为寒湿痹阻型AS的主要中医证候疗效优于单用西药组,改善关节炎症血清ESR、CRP参数,且不良反应发生率小,安全性好。Objective： To observe and evaluate the clinical effect of applying wenbi power hot compresses to ankylosing spondylitis. Methods： 56 cases of patients with ankylosing spondylitis were randomly divided into experimental group and control group,with 28 cases in each group. Two groups were given sulfasalazine tablets,thalidomide tablets and celecoxib capsules orally. Meanwhile,a hot compress of bootstrap wenbi power was topically applied in experimental group. After 4 weeks of treatment,the observed cases were evaluated as follows： the scores of low back pain at night and overall low back pain,the duration of early morning stiffness,activity of lower back（ distance from middle-finger to floor,schober experiments）,the VAS（ 0 ～ 10） scores of overall condition patients evaluated and laboratory examinations as follows： erythrocyte sedimentation rate,C- reactive protein,blood count,urine routine,liver function and renal function. Results： After 4 weeks of treatment,compared with control group,the treatment in the experimental group reduced the VAS（ 0 ～ 10） scoring parameter of low back pain（ P〈 0. 01）,shortened the duration of early morning stiffness（ P 〈0. 01）,improved the serum parameters of ESR、CRP（ P〈 0. 01）; but the effects of distance from middle-finger to floor,schober experiments and the evaluating scores of VAS（ 0 ～ 10） were not evident（ P ﹥ 0. 05）. The rate of adverse events did not differ between the two groups（ P﹥ 0. 05）. Conclusion： The efficacy of major TCM syndrome of integrated traditional Chinese medicine and Western medicine therapy by applying wenbi power hot compresses for AS which was cold- dampness blocking collaterals was better than using western medicine alone,which can improve the serum parameters of ESR、CRP,has low rate of adverse reactions and to be safe.
