运动平板与直立倾斜试验实验筛查晕厥原因

摘　　要：目的协助诊断血管迷走性晕厥。方法对患者先行基础倾斜试验,阴性者再予以舌下含服硝酸甘油继续倾斜试验。直立倾斜试验阳性者,隔日或择日行运动平板试验。结果 HUTT阳性23例,占实验总人数的76.7%。其中BHUT阳性仅2例,占阳性人数的8.7%,占实验总人数的6.7%。NHUT阳性21例,占阳性人数的91.3%,占实验总人数的70%。HUTT阴性7例,占实验总人数的23.3%。BHUT总时间为45min。BHUT阳性反应出现时间在倾斜2min。NHUT阳性反应出现时间在用药后2-12min。运动平板试验10例均为阴性。结论直立倾斜试验对血管迷走性晕厥诊断明确,运动平板试验阴性的血管迷走性晕厥患者可适度运动。Objective To assist the diagnosis of vasovagal syncope. Metheds To perform basic tilt table test on the patients at first,and continue with tilt test on the ones whose testing results are negative by sublingualing nitroglycerin. As to the patients whose testing results are positive,we will perform treadmill exercise testing on them the next day or some day later. Results 23 cases of HUTT are positive,that’s 76.7%of the total number of the experiments. Only 2 cases of BHUT are positive among them, that’s 8.7%. NHUT,21cases,that’s 91.3%. 7 cases of HUTT are negative,that’s 70% of the total number. The total test time of BHUT is 45 minutes,and the positive reaction of BHUT comes 2 minutes after the head-up tilt testing,while the positive reaction of NHUT comes 2 to 12 minutes after sublingualing nitroglycerin. TET,10 cases are all negative,that means 100% are negative. Conclusion The head-up tilt test on vasovagal syncope diagnosed clearly, the patients with vasovagal syncope whose treadmill ex-ercise test results are negative can do exercise moderately.
