摘  要:目的探讨负压封闭引流技术对儿童急性创面新生肉芽组织内血管生成的影响。方法选取因外伤所致四肢皮肤软组织撕脱及软组织缺损患儿40例,创面均在5cm×5cm大小以上,6-8h内入院并急诊行清创治疗,在Ⅰ期行彻底清创治疗后均分为2组,每组各20例,实验组采用负压封闭引流技术处理,对照组采用传统的换药治疗。分别于术后第7d、第14d取创缘新生肉芽组织用免疫组织化学染色法(S-P法),观察新生微血管数的变化。结果实验组在使用负压封闭引流技术治疗后CD34标记的新生微血管数值在7d、14d均高于对照组,差异有统计学意义(P<0.01);实验组及对照组的新生微血管数值在14d均高于7d,差异均有统计学意义(P<0.05)。结论负压封闭引流技术与常规换药相比,能够更好的促进创面微血管生长。并能够对儿童大面积皮肤软组织损伤创面起到更加积极的疗效。Objective To investigate the effect of vacuum sealing drainage (VSD) on angiogenesis of new granulation tissue on child acute wound. Methods 40 children with soft tissue avulsion of extremities resulting from injuries were selected ,and wound size were above 5cm&#215;5cm. They were hospitalized within 6-8 hours after injury treated with emergency debridement. Then the pa-tients were divided into 2 groups with 20 cases in each group. The experimental group were treated by VSD ,while the control group received common dressing convertional therapy. New granulation tissues were taken on the 7th and 14th d respectively after traumatism and stained with S-P immunohistochemistry method and observed the Micro vessel density (MVD) change. Results The MVD values marked by CD34 on the 7th and 14th in dexperimental group after VSD treatment were higher than those of con-trol group,and there were a significant difference (P〈0.01);MVD value on the 14th d after operation was higher than that of on the 7th d in both experimental and control groups,and there was significant difference in each group (P〈0.05). Conclusions To com-pare conventional treatment,the VSD treatment can promote the vascularization of wound obviously. And the VSD technique has positive curative effect on large injury wound of child soft tissue.
