摘 要:目的观察益肺健脾方对慢性阻塞性肺疾病(chronic obstructive pulmonary disease,COPD)模型大鼠的防治作用,从气道炎症和气道黏液高分泌角度探讨其作用机制。方法采用烟熏和大鼠气管内注入脂多糖(lipopolysaccharide,LPS)的方法建立COPD模型。将雄性SD大鼠随机分为空白对照组(对照组)、模型组、益肺健脾方组(中药组),每组6只。采用肺功能仪检测大鼠用力肺活量(forced vital capacity,FVC)、0.1秒用力呼气容积(forced expiratory volume in 0.1 second,FEV0.1)、FEV0.1/FVC、呼气峰值流速(peak expiratory flow,PEF),HE染色观察大鼠支气管、肺组织病理变化。采用阿尔辛蓝-过碘酸雪夫(AB-PAS)染色观察气道杯状细胞。采用ELISA法检测各组大鼠支气管肺泡灌洗液(bronchoalveolar lavage fluid,BALF)中IL-8、IL-17及TNF-α的含量。采用免疫组织化学法检测大鼠气道中黏附分子1(intercellular cell adhesion molecule-1,ICAM-1)、核因子κB(nuclear factor-κb,NF-κB)、黏蛋白5AC(mucin 5AC,Muc5AC)和T0ll样受体4(Toll-like receptor 4,TLR4)蛋白的表达。采用实时荧光定量PCR(real-time quantitative PCR,real-time qPCR)检测支气管肺组织中Muc5AC和TLR4 mRNA的表达。结果模型组大鼠支气管、肺组织改变符合COPD病理表现。与模型组比较,中药组肺损伤程度减轻。与对照组比较,模型组FVC、FEV0.1、FEV0.1/FVC、PEF降低(P<0.01),BALF中IL-8、IL-17和TNF-α含量升高(P<0.01),支气管肺组织中ICAM-1、NF-κB、Muc5AC蛋白、TLR4蛋白、Muc5AC mRNA及TLR4 mRNA表达水平升高(P<0.01)。与模型组比较,中药组FVC、FEV0.1、FEV0.1/FVC、PEF升高(P<0.01,P<0.05),其余各指标均降低(P<0.01,P<0.05)。结论益肺健脾方可降低COPD模型大鼠BALF中IL-8、IL-17、TNF-α水平,抑制气道和肺组织内ICAM-1、NF-κB、Muc5AC和TLR4蛋白的表达,通过减轻气道炎症和气道黏液高分泌作用达到防治COPD目的。Objective To observe preventive and therapeutic effect of Yifei Jianpi Recipe (YJR) on chronic obstructive pulmonary disease (COPD) model rats and to explore its mechanism from the way of airway inflammation and airway mucus hypersecretion. Methods The COPD rat model was established by using cigarette smoking combined with intratracheal injection of lipopolysaccharide (LPS). Male SD rats were randomly divided into the blank control group (control group), the model group, the Y JR group, 6 in each group. Forced vital capacity (FVC), forced expiratory volume in 0.1 second (FEV0.1), FEV0.1/FVC, peak expiratory flow (PEF) was tested by lung function device. Pathological changes of bronchi and lung tissues were observed by HE staining. Airway Goblet cells were observed using AB-PAS staining. Contents of IL-8, IL-17, and TNF-α in bronchoalveolar lavage fluid (BALF) were measured by enzyme-linked immunosorbent assay (ELISA). Protein expressions of intercellular cell adhesion molecule-1 ( ICAM-1 ), nuclear factor κB ( NF-κB), mucin 5AC (Muc5AC), and Toll-like receptor 4 (TLR4) in rat airway were detected by immunohistochemical assay, mRNA expressions of TLR4 and Muc5AC in bronchi and lung tissues were detected by real-time quantitative PCR (RT qPCR). Results Changes of bronchi and lung tissues in the model group rats were consistent with typical pathological manifestations of COPD. Compared with the model group, the degree of lung injury was significantly alleviated in the Y JR group. Compared with the control group, FVC, FEV0.1, FEV0. 1/FVC, and PEF were decreased (P 〈0.01 ), contents of IL-8, IL-17, and TNF-α in BALF were significantly increased (P 〈0.01 ), protein expressions of ICAM-1, NF-κB, Muc5AC, and TLR4, mRNA expression levels of Muc5AC and TLR4 in bronchi and lung tissues were also significantly increased in the model group (P 〈0.01).
