经皮球囊扩张治疗血液透析动静脉内瘘狭窄临床分析

摘　　要：目的：探讨经皮球囊扩张在治疗血液透析患者动静脉内瘘狭窄中的疗效。方法对19例动静脉瘘狭窄的患者行经皮球囊扩张血管成形治疗狭窄的动静脉瘘，通过对比扩张前后的瘘口震颤程度、血管造影所见的血管狭窄程度的变化、彩色多普勒超声、术后透析时流量变化，评价经皮球囊扩张成形治疗动静脉瘘狭窄的疗效。结果全部病例经球囊扩张成形均取得成功，术中及术后无明显并发症出现，术中造影见狭窄处的动静脉内瘘明显改善，狭窄程度均降至20%以下，术后瘘口处震颤明显增强，术后透析时流量明显增加，并达到透析所需。结论经皮球囊扩张治疗动静脉内瘘狭窄安全、有效，且操作简单，创伤小。Objective To explore the curative effect of percutaneous balloon dilation in treatment of arteriovenous internal fistula stenosis in hemodialysis patients.Methods 1 9 patients with arteriovenous fistula stenosis were treated by the expansion of balloon angioplasty.Changes of the fistula tremor and postoperative dialysis flow,and vascular stenosis degree on angiography and color doppler ultrasound were compared between pre-and post-treatment in order to evaluate the curative effect.Results The treatments of balloon dilation in all patients were successful without any intraoperative and postoperative complications.Intraoperative angiogra-phy showed an improvement of the arteriovenous fistula stenosis with a decrease by 20%.After treatment,the fistula tremor and di-alysis flow were significantly increased,which met the need of the hemodialysis.Conclusion The treatment of arteriovenous fistula stenosis with percutaneous balloon dilatation is safe,effective,noninvasive and simple.
