摘　　要：目的探讨重型病毒型肝炎患者医院感染病原菌分布、感染部位及危险因素,为临床重型病毒型肝炎医院感染预防控制提供依据。方法回顾性分析2006年1月-2014年1月医院收治的340例重型病毒型肝炎患者临床资料,分析发生感染患者的病原菌及感染因素,数据采用SPSS 18.0软件进行统计分析。结果共发生医院感染160例,感染率47.06%;检出病原菌主要有白色假丝酵母菌、大肠埃希菌、铜绿假单胞菌、肺炎克雷伯菌及金黄色葡萄球菌,分别占32.92%、27.08%、21.25%、13.75%、5.00%;logistic回归多因素分析结果显示,年龄、肝硬化、应用抗菌药物、免疫功能低下及侵入性操作均为重型病毒型肝炎患者发生医院感染的独立危险因素(P<0.05)。结论重型病毒型肝炎患者医院感染的发生率高、病原菌种类多、感染部位广,临床治疗过程中应根据相应的危险因素规范操作避免感染的发生。OBJECTIVE To study the risk factors and pathogenic bacteria distribution and infection sites of nosoco‐mial infections for patients with severe viral hepatitis ,so as to provide the theory basis for the clinical prevention of severe viral hepatitis .METHODS The clinical data of 340 cases of patients with severe viral hepatitis who have been treated in hospital from Jan .2006 to Jan .2014 were analyzed retrospectively to find out the pathogens and infective factors for viral hepatitis .All data were statistically analyzed by SPSS 18 .0 software .RESULTS Totally 60 cases were infected in the hospital and the incidence rate was 47 .06% .Pathogens mainly included Candida al‐bicans , E .coli , Pseudomonas aeruginosa , K lebsiella , and Staphylococcus aureus , accounting for 32 .92% , 27 .08% ,21 .25% ,13 .75% ,and 5 .00% respectively .Multiple factor logistic regression analysis showed that age ,cirrhosis ,use of antibiotics ,weak immune function and invasive operation were risk factors for nosocomial infections in patients with severe viral hepatitis （P〈 0 .05） .CONCLUSION Patients with severe viral hepatitis have high hospital infection rate ,infected by various pathogens ,and with extensive infection sites .Risk factors should be taken serious consideration during clinical therapy so as to prevent occurrence of infections .
