胰腺癌患者术后腹部感染的CT诊断及鉴别

摘　　要：目的探讨胰腺癌患者术后腹部感染的CT诊断表现及鉴别诊断特点,提高诊断正确率。方法随机选取2010年12月-2013年12月经手术和病理组织切片检查证实的48例胰腺癌患者为研究对象,对其CT影像及临床资料进行回顾性分析,数据采用SPSS 17.0软件进行统计分析。结果 CT诊断为胰腺癌术后腹部机遇性感染的患者有19例,漏诊2例,分别为肠梗阻及腹腔脓肿各1例;误诊1例为化脓性腹膜炎;腹部感染CT诊断主要表现为肠梗阻性病变8例、腹腔脓肿7例及化脓性腹膜炎4例,且CT扫描均有不同程度的脓肿、肠壁增厚及腹腔积液。结论胰腺癌患者术后腹部感染的CT诊断对感染的早期确诊有着重要临床价值,有利于提高对胰腺癌术后并发症的控制,有助于做出正确的术后诊断并及时确定治疗方案。OBJECTIVE To explore the characteristics of CT diagnosis and differential diagnosis of postoperative ab‐dominal infections in the pancreatic cancer patients so as to raise the accuracy of diagnosis .METHODS From Dec 2010 to Dec 2013 ,a total of 48 patients who were confirmed as pancreatic cancer through surgical and histopatho‐logic slide examination were recruited as the study objects .The CT imaging and clinical data of the participants were retrospectively analyzed ,and the statistical analysis of data was performed with the use of SPSS 17 .0 soft‐ware .RESULTS The CT diagnosis showed that 19 pancreatic cancer patients had postoperative opportunistic ab‐dominal infections ,and 2 cases missed the diagnosis ,including 1 case of intestinal obstruction and 1 case of ab‐dominal abscess ;1 case of missed diagnosis had purulent peritonitis .The major manifestations of CT diagnosis of abdominal infections included 8 cases of obstruction lesions ,7 cases of abdominal abscess ,and 4 cases of purulent peritonitis ;the CT scans showed varying degrees of abscess ,intestinal wall thickening ,and peritoneal fluid . CONCLUSION The CT diagnosis of postoperative abdominal infections in the pancreatic cancer patients has signifi‐cant clinical value in early dialysis of the infections ,facilitates the control of postoperative complications ,and con‐tributes to the correct postoperative diagnosis so as to develop the treatment program in a timely manner .
