摘 要:目的:通过认知重评任务考察抑郁症正性情绪调节异常特点及其脑机制。方法:收集抑郁症组14名被试和对照组16名被试,并匹配其性别、年龄和文化程度,利用认知重评任务考察抑郁症正性情绪调节异常的脑功能联结特点。结果:在行为学方面,抑郁症患者能够使用认知重评策略增强和减弱正性情绪。功能磁共振成像结果显示,上调正性情绪时,抑郁症患者左侧眶额叶、右侧颞中回/颞极等区域激活低于对照组,双侧梭状回、左侧舌回、右侧小脑等区域激活高于对照组;下调正性情绪时,抑郁症患者右侧颞中回/颞极激活低于对照组。结论:抑郁症患者难以调用所需要认知资源增强和减弱正性情绪,认知控制能力下降,并表现出情绪调节相关区域(眶额叶、颞叶)活动异常。Objective: To examine the neural substrates of abnormal positive emotion regulation using cognitive reap- praisal paradigm. Methods: 14 patients and 16 controls matched with gender, age and education were recruited, and under- went reappraisal task during fMRI scanning in a 3.0T scanner. Imaging data were preprocessed and analyzed with SPM 8 software. Results: When up regulating positive emotion, depressed patients showed reduced activity in the orbito-frontal cortex, right temporal gyrus/temporal pole and increased activity in the bilateral fusiform gyrus, left lingual gyrus and right cerebellum. In addition, depressed patients failed to recruit the right middle temporal gyrus/temporal pole to down-regulate positive emotion. Conclusion: Depressed patients have deficits in cognitive control function, which may lead to their inabil- ity to recruit cognitive control resources to enhance and reduce positive emotion. This cognitive deficit may be underlied by abnormal functions in the orbital-frontal gyrus and temporal gyrus associated with emotion regulation.
