p16、p53、Ki-67在宫颈上皮内病变诊断中的临床病理意义

摘　　要：目的探讨p16、p53及Ki-67在不同程度宫颈上皮内病变诊断中的临床病理意义。方法选取2012年1月至2014年6月期间成都市妇女儿童中心医院150例患有宫颈鳞癌、宫颈上皮内瘤变(cervical intraepithelial neoplasia,CIN)以及正常宫颈的组织,用免疫组化SP法检测其p16、p53及Ki-67表达。结果 p16、p53、Ki-67在宫颈鳞癌组织中的阳性表达率分别为100%、46.67%、100%,在CINⅢ中的阳性表达率分别为95%、27.5%、100%,在CINⅡ中的阳性表达率分别为87.5%、22.5%、100%,在CINⅠ中的阳性表达率分别为63.5%、13.33%、100%。宫颈鳞癌组、CIN组(宫颈上皮内病变)中p16、p53及Ki-67的阳性表达率与对照组比较,差异均有统计学意义(P<0.05)。p16、p53及Ki-67在CINⅠ、CINⅡ/Ⅲ及宫颈鳞癌的表达强度逐渐增加,且与CIN分级呈正相关。结论 p16、p53及Ki-67这3种相关蛋白作为辅助的诊断标记物对宫颈病变的诊治有参考价值。Objective investigation the pathological significance of p53, Ki -67 and pl6 in diagnosis of cervical intraepithelial lesions. Methods Cervical tissues of 150 cases with squamous cell carcinoma of the uterine cervix, cervical intraepithelial neoplasia （CIN） and normal cervical were extracted in Chengdu Women and Children＇s Central Hospital from Jan 2012 to Jun 2014, pl6, p53 and Ki-67 were detected by immunohistochemical SP method. Results The expression positive rates of pl6, p53 and Ki -67 in cervical squamous cell carcinoma group were 100 %, 46. 67 %, 100 % respectively, those in CIN III were 95 %, 27. 5 % and 100% respectively, inCINII were 87.5 %, 22.5 %, 100 %, in CIN I were 63.5 %, 13.33 %, 100 %. The differences of expression positive rates of pl6, p53 and Ki - 67 between cervical squamous cell carcinoma group, cervical intraepithelial neoplasia group and control group had statistical significant （P 〈 0.05 ）. The expressional intensity of pl6, p53 and Ki - 67p16 increased in CIN and cervical squamous cell carcinoma, which were positively associated with CIN degree. Conclusion The protein p16, P53 and Ki -67 have assisted reference value for cervical lesions as a diagnostic marker.
