蒙特利尔认知评估量表在轻度认知功能障碍诊断中的应用

摘　　要：目的:探讨蒙特利尔认知评估量表北京版(Montreal cognitive assessment scale,Mo CA)在轻度认知功能障碍(mild cognitive impairment,MCI)诊断中应用的可行性。方法:选择MCI患者58例(MCI组),另选同期健康体检者51例(对照组)。分别给予Mo CA和简易智能状态检查量表(MMSE)评估,并分析评估结果。结果:MCI组MoCA总分及MMSE总分均低于对照组(t=-10.544,-16.188;P<0.01)。与对照组比较,MCI组Mo CA总分及各亚项评分明显降低(t=-7.625,-4.614,-5.306,-7.505,-5.991,-7.405,-3.020;P<0.01)。Mo CA诊断MCI的敏感性为96.55%,特异性为76.47%,MMSE诊断MCI的敏感性为55.17%,特异性为96.07%。结论:Mo CA用于MCI诊断时敏感性优于MMSE,能全面评估MCI患者的认知功能。Objective：To study the application of Beijing version Montreal cognitive assessment scale （MoCA） in diagnosis of mild cognitive impairment in elderly patients. Methods： Fifty - eight patients with MCI and fifty - one healthy subjects were examined with neuropsychological tests using MoCA and Mini - mental state examination（MMSE）. Results：The total scores of MoCA and MMSE in the MCI group were significantly lower than the scores in the control group（ t = -10. 544 ,-16. 188 ;P 〈 0.01 ）. The sub - item scores on the MoCA scale were significantly lower in MCI group than in control group （t =-7. 625, -4. 614,-5. 306,-7.505 ,-5.991 ,-7.405 ,-3.020 ;P 〈 0.01 ）. The sensitivity and specificity of MoCA and MMSE scales were 96.55% and 76.47% ,55.17% and 96.07% ,respectively,for the detection of MCI. Conclusion ： The sensitivity of MoCA is higher than that of MMSE in assessment of MCI. It can be used to assess cognitive function in patients with MCI comprehensively.
