摘  要:目的观察重症手足口病患儿地塞米松治疗疗效及治疗前后细胞因子水平变化。方法将2011~2012年在我院诊断为重症手足口病患儿100例,随机分为A、B两组,所有入选病例均予利巴韦林10mg/(kg·d),A组予0.1mg/(kg·d)地塞米松+1g/(kg·d)人免疫球蛋白,总量为2g/kg治疗,B组予1g/(kg·d)人免疫球蛋白,总量为2g/kg治疗。结果 A组总有效49例(98%);B组总有效38例(76%)。两组总有效率差异有统计学意义(P=0.001〈0.05)。两组患儿治疗后细胞因子IL-10、TGF-β、IL-35水、TNF-α、IFN-γ、IL-17水平,分别与入院时相比较,差异均具有统计学意义(均P〈0.05)。结论 EV71感染所致手足口病,不仅仅是病毒对机体的直接作用,可能与病毒感染后促发的全身炎症反应有关。Objective To observe the curative effect of dexamethasone for the severe hand foot and mouth disease in children and the changes of cytokine level before and after the treatment. Methods 100 cases of children diagnosed as severe hand foot and mouth disease in our hospital from 2011 to 2012, were randomly divided into A, B two groups, all patients were given the Leigh Bhave Lin 10mg/(kg·d) for treatment, A group of patients were given 0.1mg/(kg·d) + 1g/(kg·d) dexamethasone Human Immunoglobulin, total dose was 2g/kg for treatment, B group were given 1g/(kg·d) Human Immunoglobulin, a total dose of 2g/kg. Results The total effectiveness of a group was 49 cases (98%);38 cases of B group(76%). The total efficiency of two groups had significant difference (P=0.001〈0.05). The levels of the cytokines IL-10, TGF-beta, IL-35 water, TNF-alpha, IFN-gamma, IL-17 of the two groups after treatment were significant difference to all that levels before treatment(P〈0.05). Conclusion The cause of the hand foot and mouth disease was EV71 infection, it’s not only a direct action of the virus on the body, but also a promoted systemic inflammatory response by virus infection.
