摘 要:目的:探讨不同剂量枸橼酸咖啡因治疗新生儿呼吸暂停的临床疗效。方法选择2013年1月~2014年7月本院收治的呼吸暂停新生儿82例作为研究对象,随机分为两组,每组41例,两组均采用枸橼酸咖啡因治疗新生儿呼吸暂停,观察组首次剂量为20mg/kg,静脉滴注,维持量为10mg/kg 持续静脉滴注,对照组按照传统的剂量首次剂量为20mg/kg,静脉滴注,维持量为5mg/kg 持续静脉滴注,直至患儿停止呼吸暂停1周,比较两组患儿的治疗有效率和呼吸暂停的发作持续时间、发作频次、症状消失时间。结果观察组总有效率高于对照组,两组比较差异有统计学意义(P <0.05);观察组患儿的呼吸暂停在治疗结束后的发作持续时间、发作频次、症状消失时间均少于对照组,两组比较差异有统计学意义(P <0.05),两组患儿在治疗过程中未见不良反应,且对心率血压影响无差异。结论枸橼酸咖啡因以10mg/kg 维持量治疗新生儿呼吸暂停疗效显著,有利于患儿早期康复。Objective To evaluate the curative effect of different doses of caffeine citrate in the treatment of apnea neonatorum. Methods 82 patients with apnea neonatorum who were admitted to our hospital from January 2013 to July 2014 were selected as research subjects and were randomly allocated to the observation group and the control group,with 41 in each.Patients with apnea neonatorum in two groups were both received caffeine citrate.Patients in the observation group were received first dose of 20mg/kg by intravenous drip and maintenance dose of 10mg/kg by continuous infusion while patients in the control group were received first dose of 20mg/kg by intravenous drip and maintenance dose of 5mg/kg by continuous infusion until one week after the patients stopped apnea.Effective rate of treatment,episode duration of apnea,episode frequency and symptom disappearance time of the two groups were compared. Results Total effective rate in the observation group was higher than that in the control group and the difference was statistically significant(P 〈 0.05).Episode duration, episode frequency,symptom disappearance time of apnea after the treatment in the observation group were all less than those in the control group and the difference was statistically significant(P 〈 0.05).No adverse reaction appeared in the treatment and there was no difference in the effect on heart rate and blood pressure in two groups. Conclusion Maintenance dose of 10mg/kg of caffeine citrate in the treatment of apnea neonatorum has a significant curative effect and it is useful for patients' early rehabilitation.
